PDCA联合PBL教学法降低医学生血源性职业暴露发生率的效果研究

Study on the Effect of PDCA Combined With PBL Teaching Method on Reducing the Incidence of Blood-Borne Occupational Exposure in Medical Students

摘要 目的 探讨PDCA即计划(plan)-实施(do)-检查(check)-处理(act)联合以问题为导向的教学法(problem-based learning,PBL)在医学实习生血源性职业暴露防护中的应用,降低医学实习生血源性职业暴露发生率。方法 选取2020年8月—2021年8月于广西医科大学第二附属医院实习的实习生529名,根据是否实施PDCA联合PBL教学法分为两组,2020年8月—2021年2月的实习生为对照组(n=214),2021年3—8月的实习生为研究组(n=315),比较两组的血源性职业暴露知晓率、血源性职业暴露防御意识和行为依从性及血源性职业暴露发生率。结果 研究组实习生血源性职业暴露知晓率、血源性职业暴露防御意识和行为依从性均高于对照组,差异有统计学意义(P <0.05);研究组实习生血源性职业暴露发生率低于对照组,差异有统计学意义(P<0.05)。结论 PDCA联合PBL教学法能有效提升医学实习生血源性职业暴露防护能力,对降低医学实习生血源性职业暴露发生率及养成良好的职业习惯有积极作用。 Objective To explore the application of plan-do-check-act(PDCA)combined with problem-based learning(PBL)teaching method in the prevention of blood-borne occupational exposure of medical interns,so as to effectively reduce the incidence of blood-borne occupational exposure of medical students.Methods A total of 529 interns who practiced in the Second Affiliated Hospital of Guangxi Medical University from August 2020 to August 2021 were selected and divided into two groups according to whether the PDCA combined with PBL teaching method was implemented.The interns from August 2020 to February 2021 were included in the control group(n=214),and the interns from March to August 2021 were included in the research group(n=315).The awareness rate of blood-borne occupational exposure,the awareness of blood-borne occupational exposure,the compliance of behavior and the incidence of blood-borne occupational exposure were compared between the two groups.Results The awareness rate of blood-borne occupational exposure,defense awareness of blood-borne occupational exposure and behavioral compliance of interns in the research group were higher than those in the control group,and the differences were statistically significant(P<0.05).The incidence of blood-borne occupational exposure in the research group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion PDCA combined with PBL teaching method is a scientific management mode,which can effectively improve the occupational protection ability of medical interns against blood-borne occupational exposure,and play a positive role in reducing the incidence of blood-borne occupational exposure of medical interns and developing good and healthy occupational habits.
