摘要
目的:研究采用冷侧压法、热牙胶垂直加压法以及单尖法对根管治疗患者预后的影响,进一步为临床治疗牙体牙髓病根管治疗的填充选择提供依据。方法:回顾性分析2021年2月—2022年2月中国人民解放军联勤保障部队第九八八医院(焦作院区)收治的140例行根管治疗的患者临床资料。根据根管治疗时填充根管方法的不同分为3组。其中39例患者采用冷侧压法填充根管作为A组;55例患者采用热牙胶垂直加压法填充根管作为B组;46例患者采用单尖法填充根管作为C组。观察3组患者根管填充效果、治疗效果、血清炎症因子以及并发症情况。结果:C组填充效果明显优于A组和B组,差异有统计学意义(χ^(2)=9.540,P<0.05);A组、B组和C组治疗总有效率比较,差异无统计学意义(χ^(2)=0.500,P>0.05);治疗后,C组IL-4水平明显高于A、B组,IL-6和TNF-α水平明显低于A、B组,差异有统计学意义(F=12.770、24.210、6.420,P<0.05);A组、B组和C组患者并发症包括牙周疼痛、根尖疼痛以及牙龈出血比较,差异无统计学意义(χ^(2)=4.678,P>0.05)。结论:3种根管填充方式治疗牙体牙髓病效果均较好,其中单尖法填充根管能有效缓解牙周炎症反应,填充效果更佳。
Objective:The effects of using cold lateral pressure method,hot cement vertical pressure method,and single-tip method on the prognosis of endodontically treated patients to further provide a basis for filling selection for clinical treatment of endodontic endodontics.Methods:The clinical data of 140 patients admitted to the hospital for root canal treatment from February 2021 to February 2022 were retrospectively analyzed.After analyzing the patients’clinical history data,they were divided into three groups according to the different methods of filling root canals during endodontic treatment,in which 39 patients were filled with cold lateral pressure method as group A,55 patients were filled with hot glue vertical pressure method as group B,and 46 patients were filled with single-tip method as group C.The root canal filling results,treatment outcomes,serum inflammatory factors,and complications were observed in the three groups of patients.Results:The filling effect in group C was significantly better than that in groups A and B,and the difference was statistically significant(χ^(2)=9.540,P<0.05).There was no statistically significant difference between the total treatment efficiency of group A,group B and group C(χ^(2)=0.500,P>0.05).After treatment,IL-4 levels in group C were significantly higher than those in groups A and B,and IL-6 and TNF-αlevels were significantly lower than those in groups A and B,and the differences were statistically significant(F=12.770,24.210,6.420,P<0.05).Complications in patients in groups A,B and C included periodontal pain,apical pain and gingival bleeding,and the difference was not statistically significant in all three groups compared(χ^(2)=4.678,P>0.05).Conclusion:All three root canal filling methods have better results in treating endodontic diseases,among which the single-tip method of filling root canals can effectively alleviate periodontal inflammatory reactions and provide better filling results.
