摘要
目的探讨室性早搏(简称室早)的频度与心率的相关性及临床特征。方法根据室早频度与心率的相关性,将室早分为心率正相关性、负相关性和非相关性,分析不同类型室早的患者特点、室早特征及起源部位的差异。结果共纳入特发性室早患者142例,其中心率正相关性58例、心率负相关性30例和心率非相关性54例。心率负相关性的患者年龄显著低于其他两种类型[(45±20)岁vs(55±14)、(55±14)岁,P<0.05],24 h平均心率显著高于其他两种类型[(79±8)次/分vs(72±8)、(75±9)次/分,P<0.05]。心率负相关性的夜间室早频度最高,平均为白天室早频度的1.75倍,心率非相关性的夜间、白天室早频度接近,心率正相关性的夜间室早频度低于白天。室早起源部位在三种类型室早之间无显著差异。结论与心率正相关和非相关两种类型相比,心率负相关性室早的患者年龄较轻、平均心率较快;三种类型室早的昼夜分布存在显著差异。
Objective To investigate the relationship between heart rate(HR)and density of premature ventricular complex(PVC)in idiopathic PVC patients and the related clinical features.Methods PVCs were divided into PVC positively related with HR(POS PVC),PVC negatively related with HR(NEG PVC)and PVC not related with HR(NR PVC)according to the correlation between hourly PVC density and HR.Patient characteristics,PVC features and PVC origins of the three PVC types were analyzed.Results One hundred and forty-two patients with idiopathic PVC were included,including 58 cases of POS PVC,30 cases of NEG PVC and 54 cases of NR PVC.Compared with POS PVC and NR PVC,the patients with NEG PVC were significantly younger[(45±20)years vs(55±14),(55±14)years,P<0.05],and their average HR of 24 hours was significantly increased[(79±8)bpm vs(72±8),(75±9)bpm,P<0.05].NEG PVC occurred more frequently at night with a PVC density averagely 1.75 times of that during daytime.The PVC densities of NR PVC during daytime and nighttime were similar.And POS PVC showed a decreased PVC density at night when compared with daytime.No differences of PVC origins were found among the three types of PVC.Conclusion Compared with POS PVC and NR PVC,patients with NEG PVC are younger and have increased average HR.Different circadian distributions of PVCs are observed among the three PVC types.
