摘要
目的 探讨MWA联合CEUS在BTNS治疗中的应用价值。方法 回顾性分析2019年9月-2022年8月于云南省滇南中心医院(红河州第一人民医院)超声医学科接受MWA治疗的40例少数民族患者病例资料;术前行FNA筛选BTNS,术中行MWA联合CEUS对BTNS消融,术后行CEUS对消融灶情况评价,随访BTNS在治疗前后体积变化等。结果 术前病理证实40例TN均为BTNS(n=49);术中首次完全消融率为91.80%(45/49);术后随访复发率为0%,1、3、6个月随访VRR分别为0.09(0.35,0.46)、0.35(0.71,0.21)、0.77(0.89,0.45),消融灶体积均明显缩小(P<0.05),并发症发生率为2.50%(1/40)。结论 MWA联合CEUS治疗BTNS是一种安全有效的方法,值得临床广泛推广。
Objective To investigate the The application value of microwave ablation(MWA)combined with contrast enhanced ultrasound(CEUS)in treatment of benign thyroid nodulesic(BTNS).Methods The medical data of 40 ethnic minority patients who received MWA treatment in the Department of Ultrasound Medicine of Southern Central Hospital of Yunnan Province/First People s Hospital of Honghe Prefecture from September 2019 to August 2022 were retrospectively analyzed.BTNS patients were screened by fine needle aspiration biopsy(FNA)before surgery.BTNS were ablated by MWA combined with CEUS during surgery.The ablation focus were evaluated by CEUS after surgery,and BTNS volume changes before and after treatment were followed up.Results All TN of 40 cases were verified as BTNS by pathology(n=49).The first complete ablation rate was 91.80%(45/49)in the operation.The followe-up recurrence rate was 0%,volume reduction rate(VRR)at 1,3 and 6 months follow up was 0.09(0.35,0.46),0.35(0.71,0.21)and 0.77(0.89,0.45),respectively,and the volume of ablation sites decreased significantly(P<0.05),the complication rate was 2.50%(1/40).Conclusions MWA combined with CEUS in the treatment of BTNS is a safe and effective method,which is worthy of widespread clinical promotion.
