摘要
目的探讨小儿鼾症采用扁桃体摘除与鼻内镜低温等离子刀扁桃体腺样体切除术联合治疗的临床效果。方法选取2021年5月—2023年5月莒县人民医院收治的共计86例鼾症患儿,以随机数表法分成研究组(n=43)与对照组(n=43),两组均扁桃体摘除术,对照组联合常规腺样体刮除术,研究组联合鼻内镜低温等离子刀扁桃体腺样体切除术,比较两组临床指标、疼痛程度、呼吸功能、睡眠质量及并发症。结果研究组术中出血量较对照组更少,手术、住院时间更短,差异有统计学意义(P<0.05);研究组术后24、48、72 h儿童疼痛程度评分较对照组更低,差异有统计学意义(P<0.05);术后研究组各项呼吸功能指标水平均优于对照组,差异有统计学意义(P<0.05);术后研究组睡眠质量评分较对照组更低,差异有统计学意义(P<0.05);研究组并发症发生率为6.98%,与对照组的23.26%相比更低,差异有统计学意义(χ^(2)=4.441,P<0.05)。结论扁桃体摘除与鼻内镜低温等离子刀扁桃体腺样体切除术联合治疗小儿鼾症,能够促进围手术期指标改善,缓解疼痛程度,改善呼吸功能与睡眠质量,降低并发症发生率。
Objective To discuss the clinical effects of combined treatment of pediatric snoring with tonsil removal and nasal endoscopic low-temperature plasma knife tonsil adenoidectomy.Methods A total of 86 snoring children admitted to Juxian People's Hospital from May 2021 to May 2023 were selected and divided into a study group(n=43)and a control group(n=43)by randomized numerical table method.Both groups underwent tonsil removal,the control group combined with conventional adenoid scraping,and the study group combined with nasal endoscopic lowtemperature plasma knife tonsil adenoidectomy.Clinical indexes,pain level,respiratory function,sleep quality and complications were compared between the two groups.Results The intraoperative blood loss in the study group was less than that in the control group,and the duration of operation and hospitalization was shorter,the difference was statistically significant(P<0.05).The pain degree score of the study group were lower than those of the control group 24,48 and 72 h after operation,and the difference was statistically significant(P<0.05).The level of respiratory function indexes in the study group was better than that in the control group after surgery,and the difference was statistically significant(P<0.05).The sleep quality score of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the study group was 6.98%,which was lower than 23.26%in the control group,and the difference was statistically significant(χ^(2)=4.441,P<0.05).Conclusion The combination of tonsil removal and nasal endoscopic low-temperature plasma knife tonsil adenoidectomy in the treatment of pediatric snoring can promote the improvement of perioperative indexes,alleviate the degree of pain,improve the respiratory function and quality of sleep,and reduce the complication rate.
