血清BMP2表达水平与帕金森病的相关性研究

摘要 目的 探讨骨形态发生蛋白2(Bone morphogenetic protein 2,BMP2)表达水平与帕金森病(Parkinson′s disease, PD)的相关性。方法 2019年1月-2022年12月在新疆医科大学第二附属医院神经内科门诊就诊的178例PD患者为PD组;来自相同地区且同一时间段无PD临床表现及PD家族史,并在性别、年龄上相互匹配的185例健康体检者为对照组。比较两组BMP2和α突触核蛋白(α-synuclein, α-syn)的表达水平差异。采用两变量的线性相关性分析,探讨BMP2和α-syn的相关性。采用受试者工作特征(ROC)曲线评估BMP2和α-syn表达水平对PD的预测价值。采用二元Logistic回归分析评估BMP2对α-syn的影响。比较分析BMP2表达水平在PD组不同Hoehn-Yahr分期和简易精神状态检查量表(Mini-mental state examination, MMSE)评分间的差异。结果 PD组BMP2表达水平低于对照组(t=-5.582,P<0.001),而PD组α-syn表达水平高于对照组,差异有统计学意义(t=3.669,P=0.001)。相关性分析结果显示,BMP2和α-syn呈负相关(r=-0.351,P<0.001)。ROC分析结果显示,BMP2的曲线下面积(AUC)(95%CI)为0.599(0.540~0.657)。α-syn的曲线下面积(AUC)(95%CI)为0.658(0.602~0.714)。二元Logistic回归分析结果显示,BMP2可影响α-syn表达水平,即高水平的BMP2对抑制α-syn的升高有保护作用(OR=0.951,P=0.001)。随着Hoehn-Yahr分期的增高,PD组患者BMP2表达水平逐渐下降,差异有统计学意义(F=24.956,P<0.001);PD组患者不同MMSE评分之间的BMP2表达水平差异无统计学意义(F=1.138,P=0.323)。结论 BMP2表达水平与PD的进展有关,且高水平的BMP2对于抑制α-syn的升高有保护作用。 Objective To explore the correlation between serum bone morphogenetic protein 2(BMP2)expression level and Parkinson′s disease(PD).Methods 178 PD patients were collected and included in PD group.During the same period,185 healthy subjects matched for age and gender were recruited as control group.The expression levels and baseline data of BMP2 andα-synuclein(α-syn)between the 2 groups were compared,and linear correlation analysis of two variables was used to explore the correlation between BMP2 andα-syn levels.Subject operating characteristic(ROC)curves were used to assess the predictive value of BMP2 andα-syn expression levels for PD.Finally,two-variable linear correlation analysis was used to explore the correlation between BMP2 and the Hoehn-Yahr stage and Mini-Mental State Examination(MMSE)score in PD group.ResultsThe serum BMP2 content of the PD group was lower than that of control group(t=-5.582,P<0.001),while the peripheral bloodα-syn content of PD group was higher than that of control group,and the difference was statistically significant(t=3.669,P=0.001).Correlation studies showed that BMP2 andα-syn expression were negatively correlated(r=-0.351,P<0.001).ROC analysis results showed that the area under the curve(AUC)(95%CI)of BMP2 was 0.599(0.540~0.657).The area under the curve(AUC)(95%CI)ofα-syn was 0.658(0.602~0.714).The results of binary multivariate Logistic regression analysis showed that BMP2 can affectα-syn levels,that was,high levels of BMP2 had a protective effect on inhibiting the increase ofα-syn(OR=0.951,P=0.001).As Hoehn-Yahr stage was increased,the expression level of BMP2 gradually was decreased,and the difference was statistically significant(F=24.956,P<0.001).There was no statistically significant difference in BMP2 expression levels among the normal MMSE score group,mild group and moderate-severe group(F=1.138,P=0.323).ConclusionThe concentration of BMP2 in serum is related to the progression of PD,and high levels of BMP2 have a protective effect on inhibiting the increase ofα
