摘要
目的了解厦门医学院附属第二医院儿科病区医务人员手卫生现状及影响因素,为进一步提高手卫生依从性、正确性管理措施提供依据。方法2021年9月13—24日调查厦门医学院附属第二医院儿科病区医师、护士、工勤人员手卫生依从性及正确性等情况。结果共观察医院儿科病区医务人员应执行手部卫生224人次,医务人员手部卫生依从率为74.55%(167/224),3种不同职业中依从率最高的是护士,工勤人员最低,两者差异有统计学意义(P<0.05)。医务人员手卫生正确率为65.27%(109/167),护士为78.02%,医生为51.43%、工勤人员为33.33%,差异有统计学意义(P<0.05)。5个手卫生指征依从率由低到高依次为接触患者周围环境后(58.62%)、接触患者前(67.07%)、接触患者后(82.71%)、清洁/无菌操作前(84.61%)、接触患者体液后(100%)。造成洗手和卫生手消毒不规范的主要原因分别是未使用洗手液或皂液(43.75%)、步骤不全(64.29%)。结论医务人员接触患者周围环境后手部卫生依从性不高、洗手不用洗手液或皂液、卫生手消毒步骤不全是手卫生3个薄弱环节;工勤人员是薄弱人群;为下一阶段手卫生管理提出新要求。
Objective To understand the current situation and influencing factors of hand hygiene among medical staff in a pediatric ward of the Second Affiliated Hospital of Xiamen Medical College,and to provide a basis for further improving hand hygiene compliance and accuracy.Methods From September 13^(th) to 24^(th),2021,investigate the compliance and correctness of hand hygiene among physicians,nurses,and staff in pediatric wards of the Second Affiliated Hospital of Xiamen Medical College.Results A total of 224 medical staff in the pediatric ward of the hospital were observed to perform hand hygiene,with a compliance rate of 74.55%(167/224).Among the three different professions,the highest compliance rate was among nurses and the lowest was among workers,with a statistically significant difference(P<0.05).The accuracy rate of hand hygiene among medical staff was 65.27%(109/167),nurses 78.02%,doctors 51.43%,and workers 33.33%.The difference was statistically significant(P<0.05).The compliance rate of the five hand hygiene indicators,from low to high,was ranked as follows,after contact with the patient's surrounding environment(58.62%),before contact with the patient(67.07%),after contact with the patient(82.71%),before cleaning/aseptic procedures(84.61%),and after contact with the patient's body fluids(100%).The main reasons for non-standard hand washing and hand hygiene disinfection are the lack of use of hand sanitizer or soap(43.75%)and incomplete steps(64.29%).Conclusion Medical staff who come into contact with the patient's surrounding environment have low compliance with hand hygiene,do not use hand sanitizer or soap when washing hands,and incomplete disinfection steps for hygiene hands,which are three weak links in hand hygiene.
