改良单侧双通道脊柱内镜下腰椎椎间融合术治疗单阶段腰椎间盘突出症疗效分析

摘要 目的:分析改良单侧双通道脊柱内镜技术治疗单阶段腰椎间盘突出症的临床疗效。方法:回顾性分析江西中医药大学附属医院2018年9月—2021年6月收治的L4/L5或L5/S1椎间盘突出患者38例,随机分为观察组18例和对照组20例。观察组选用改良单侧双通道内镜下腰椎椎间融合术(ULIF)治疗,对照组采用微创经椎间孔入路腰椎椎间融合术(MIS-TLIF)治疗。记录2组手术及住院时间、切口长度、出血量、植骨融合率情况等。手术效果按视觉疼痛模拟评分(VAS)、Oswestry功能障碍指数问卷表评分(ODI)进行评定。结果:观察组手术时间较对照组长(P<0.05)。观察组住院时间、出血量、手术切口长度均少于对照组(P<0.05)。术后第1天观察组VAS评分及ODI指数较对照组低,差异有统计学意义(P<0.05);术后3、6个月及末次随访,2组VAS评分及ODI指数比较差异无统计学意义(P>0.05)。结论:改良ULIF是一种具有脊柱微创属性并在手术操作方式上加以创新的新技术,相较于MIS-TLIF视野更清晰、安全性较高、切口小、出血少、术后恢复快,可以作为治疗单阶段腰椎间盘突出症的一种选择方案,值得进一步拓展及研究。 Objective:To investigate the clinical effect of our hospital's modified unilateral biportal spine endoscopy technique in the treatment of single-stage lumbar disc herniation.Methods:A retrospective analysis of 38 patients with L4/L5 or L5/S1 intervertebral disc herniation admitted to our hospital from September 2018 to June 2021,Randomly divided into observation group and control group.The observation group was treated with modified unilateral dual-channel endoscopic lumbar interbody fusion(UBE-LIF hereinafter referred to as ULIF),and the control group was treated with minimally invasive transforaminal lumbar interbody fusion(MIS-TLIF).Record operation and hospitalization time,incision length,blood loss,and incidence of bone graft fusion rate of the two groups.The surgical effect was evaluated according to the Visual pain Simulation Scale(VAS)and Oswestry Dysfunction Index Questionnaire(ODI).Results:The operation time in the ULIF group was longer than that in the MIS-TLIF group(P<0.05).The length of hospital stay,blood loss,and incision length in the ULIF group were less than those in the MIS-TLIF group(P<0.05).On the first day after operation,the VAS score and ODI index of the ULIF group were lower than those of the MIS-TLIF group,and the difference was statistically significant(P<0.05).The index difference was not statistically significant(P>0.05).Conclusion:The modified ULIF technology is a new technology with minimally invasive spinal properties and innovative surgical operation methods.Compared with MIS-TLIF,the field of vision is clearer,the safety is higher,the incision is small,and the bleeding is less.Faster recovery.It can be used as an option in the treatment of single-stage lumbar disc herniation,and it is worthy of further development and research.
