摘要
目的观察针灸配合神经营养药物治疗听神经瘤术后面瘫的临床疗效。方法将68例听神经瘤术后面瘫患者按照随机数字表法分为2组,对照组34例予神经营养药物治疗,治疗组34例予针灸配合神经营养药物治疗,2组均治疗2个月。比较2组疗效;比较2组治疗前后面神经评定量表(Sunnybrook)评分、汉密尔顿焦虑量表(HAMA)评分、匹兹堡睡眠质量指数量表(PSQI)评分变化;比较2组面瘫症状缓解时间。结果治疗组总有效率97.06%(33/34),对照组总有效率79.41%(27/34),治疗组疗效优于对照组(P<0.05)。2组治疗后Sunnybrook评分较本组治疗前升高(P<0.05),HAMA评分、PSQI评分均较本组治疗前降低(P<0.05);治疗后治疗组Sunnybrook评分高于对照组(P<0.05),HAMA评分、PSQI评分均低于对照组(P<0.05)。治疗组睑裂变大、面肌瘫痪、口角㖞斜缓解时间较对照组缩短(P<0.05)。结论针灸配合神经营养药物治疗听神经瘤术后面瘫,能加快面瘫症状缓解,改善面神经功能,提高患者生活质量。
Objective To observe the clinical efficacy of acupuncture combined with neurotrophic drugs on postoperative facial paralysis of acoustic neuroma(AN).Methods A total of 68 AN patients with postoperative facial paralysis were randomly assigned into the treatment group(n=34)and the control group(n=34).All patients were managed by neurotrophic drugs,and those in the treatment group were additionally treated with acupuncture.Treated for 2 months,the aim was to compare Sunnybrook facial grading system(SFGS),Hamilton Anxiety Scale(HAMA),Pittsburgh Sleep Quality Index(PSQI),relief time of facial paralysis symptoms.The curative effect was assessed.Results The overall effective rate in the treatment group was better than that in the control group(97.06%[33/34]vs 79.41%[27/34],[P<0.05],respectively).After treatment,significantly increased SFGS,and decreased HAMA and PSQI scores were detected in the two groups(all P<0.05),and the treatment group was superior to the control group for SFGS,HAMA and PSQI scores(all P<0.05).The relief time of facial paralysis symptoms including large palpebral aperture,facial palsy,and crooked corners of the mouth was significantly shorter in the treatment group than in the control group(P<0.05).Conclusion For AN patients with postoperative facial paralysis,acupuncture combined with neurotrophic drugs can promote the relief of facial paralysis symptoms,improve facial nerve function and quality of life.
