摘要
目的在给予福州市马尾区罗星街道社区卫生服务中心收治的老年原发性高血压合并糖尿病患者应用氨氯地平治疗的基础上联合应用缬沙坦治疗,旨在探究联合用药对其糖脂代谢及血压水平的影响。方法从社区卫生服务中心选取2021年6月至2023年6月期间就诊的472例老年原发性高血压合并糖尿病患者作为研究样本,以分组后各组例数均等为原则将研究对象分为观察组、对照组,各236例,分组方法为随机数字表法。对照组予以氨氯地平治疗,观察组在对照组的基础上予以缬沙坦治疗。比较两组治疗前、治疗3个月后患者糖脂代谢水平、血压水平,并统计两组临床治疗总有效率、用药不良反应发生率。结果两组治疗前患者FPG(t=0.155)、2 hPG(t=0.319)、TC(t=0.294)、TG(t=0.375)、LDL-C(t=0.294)、HDL-C(t=0.462)、SBP(t=1.562)、DBP(t=1.071)水平对比(P>0.05);观察组治疗3个月后上述指标除HDL外,FPG(t=4.8551)、2 hPG(t=5.754)、TC(t=4.988)、TG(t=4.790)、LDL-C(t=4.919)、SBP(t=4.963)、DBP(t=4.952)水平均低于对照组(P<0.05);观察组HDL-C(t=4.913)水平高于对照组(P<0.05)。观察组、对照组临床治疗总有效率分别为96.19%、88.14%,前者高于后者(χ^(2)值为10.587,P<0.05);观察组、对照组不良反应发生率分别为1.69%、2.97%,前者低于后者(χ^(2)值为0.372,P<0.05)。结论给予社区卫生服务中心老年原发性高血压合并糖尿病患者缬沙坦联合氨氯地平治疗可以取得较佳治疗效果,有效改善其糖脂代谢水平并降低血压水平,且较少引发不良反应。
Objective To explore the effects of amlodipine combined with valsartan on glucose and lipid metabolism and blood pressure of elderly patients with essential hypertension complicated with diabetes in community outpatient clinics.Methods 472 elderly patients with essential hypertension combined with diabetes mellitus were selected from community health service center from June 2021 to June 2023.They were divided into two groups according to random number table method observation group(236 cases)and control group(236 cases).Control group was treated with amlodipine,observation group was treated with valsartan on the basis of control group.The glucose and lipid metabolism levels and blood pressure levels of the two groups were compared before treatment and after 3 months of treatment,and the total effective rate of clinical treatment and the incidence of adverse drug reactions of the two groups were analyzed.Results There were no significant differences in the levels of FPG,2 hPG,TC,TG,LDL-C,HDL-C,SBP and DBP between two groups before treatment(P>0.05).After 3 months of treatment,the levels of FPG,2 hPG,TC,TG,LDL-C,SBP and DBP in the observation group were lower than those in the control group(P<0.05).The level of HDL-C in observation group was higher than that in control group(P<0.05).The total effective rate of observation group was 96.19%,which was higher than that of control group 88.14%(P<0.05).The incidence of adverse reactions in observation group was 1.69%,which was lower than that in control group 2.97%,(P<0.05).Conclusions Valsartan combined with amlodipine in elderly patients with essential hypertension combined with diabetes in community health service center can achieve better therapeutic effect,effectively improve the level of glucose and lipid metabolism and reduce blood pressure,and rarely cause adverse reactions.
