摘要
目的探讨“管家闭环式”慢性病管理模式在基层医疗原发性高血压管理中的应用价值。方法选取2021年5月—2022年5月在承德市中心医院(三级医院)明确诊断为原发性高血压的患者100例,将其随机分为实验组和对照组,每组50例。对照组采用以电话/门诊随访的形式进行高血压常规干预,实验组以基层定点医疗机构为基础构建“管家闭环式”慢性病管理模式进行干预。分别收集管理前、管理后3个月和6个月患者的基础资料和高血压知-信-行量表信息,比较两组干预前、后的效果。结果干预3个月、6个月后,实验组的高血压治疗率和控制率明显高于对照组(P<0.05);实验组的高血压相关知识、信念与态度、行为改变,即知-信-行评分明显高于对照组(P<0.05)。结论“管家闭环式”慢性病管理模式在基层医疗原发性高血压管理中,可提高患者对原发性高血压的认知水平和治疗的依从性,提高原发性高血压的控制率和治疗率,或可有效降低高血压相关并发症。
Objective To explore application value of"housekeeper closed-loop"chronic disease management model in management of primary hypertension in grassroots healthcare.Methods The paper chose 100 patients diagnosed as essential hypertension in Chengde Central Hospital(GradeⅢHospital)from May 2021 to May 2022,and divided them into experimental group and control group randomly,with 50 cases in each group.Control group was treated with routine intervention for hypertension through telephone/outpatient follow-up,while experimental group with"housekeeper closed-loop"chronic disease management model based on grassroots designated medical institutions for intervention.Basic data and hypertension knowledge behavior scale information of patients before,3 months,and 6 months after management were collected,effect between two groups before and after intervention was compared.Results After 3 and 6 months of intervention,treatment and control rates of hypertension in experimental group were significantly higher than control group(P<0.05).Knowledge,beliefs,attitude,and behavioral changes related to hypertension in experimental group was significantly higher than control group(P<0.05).Conclusion"Housekeeper closed-loop"chronic disease management model can improve cognitive level and treatment compliance,improve control and treatment rates of primary hypertension patients,reduce hypertension related complications effectively in grassroots medical management of primary hypertension.
