摘要
银屑病是一种临床上常见的慢性鳞屑性皮肤病,角质形成细胞异常增殖是其重要的病理特征之一。肥胖是一种慢性代谢性疾病,脂肪代谢紊乱是其主要病理生理基础。银屑病与肥胖常相伴发,两者有着密切的联系。中医认为,银屑病合并肥胖主要病机为脾失健运,痰瘀互结。现代医学发现了银屑病合并肥胖与胰岛素抵抗之间的关系。胰岛素抵抗可通过影响角质形成细胞的增殖与分化、影响脂肪代谢,从而诱发银屑病和肥胖的发生。中医所提到的“痰瘀”与胰岛素抵抗之间存在密切的联系。中医认为“肥白人多痰湿”且“顽疾必兼痰和瘀”,胰岛素抵抗的形成与痰瘀的形成有着共同的原因。嗜食肥甘厚味且倦怠少动,使过多的营养物质失于运化,进而转为痰、湿、浊、脂堆积于体内,这也是胰岛素抵抗形成的主要原因。痰湿、瘀血是胰岛素抵抗发展的病理产物。另外,痰瘀还是胰岛素抵抗的临床表现特点。文章从中医角度审视胰岛素抵抗与痰湿、瘀血形成、演变的关联性,探讨中医从痰湿及瘀血论治银屑病合并肥胖的新路径,为中医治疗银屑病提供理论支持与新思路。
Psoriasis is a clinically common chronic scaly skin disease,with abnormal proliferation of keratinocytes as one of its important pathological features.Obesity is a chronic metabolic disease,and disorder of fat metabolism is its main pathophysiological basis.Psoriasis and obesity are often associated and closely related.TCM medicine believes that the main pathogenesis of psoriasis combined with obesity is the loss of spleen function and the accumulation of phlegm and blood stasis.Modern medicine has discovered the relationship between psoriasis combined with obesity and insulin resistance.Insulin resistance can induce psoriasis and obesity by affecting the proliferation and differentiation of keratinocytes,as well as affecting fat metabolism.There is a close connection between the phlegm stasis mentioned in TCM medicine and insulin resistance.TCM medicine believes that fat and white people have more phlegm and dampness and persistent diseases must have phlegm and blood stasis.The formation of insulin resistance and the formation of phlegm and blood stasis have a common cause.Poor diet and lack of exercise prevent the conversion of excessive nutrients,leading to the accumulation of phlegm,dampness,waste,and fat in the body.This is also the main reason for the formation of insulin resistance.Phlegm dampness and blood stasis are pathological products of the development of insulin resistance.In addition,phlegm and blood stasis are also clinical manifestations of insulin resistance.This article examines the correlation between insulin resistance and the formation and evolution of phlegm dampness and blood stasis from the perspective of TCM medicine,and explores a new path for treating psoriasis combined with obesity from the perspective of phlegm dampness and blood stasis,providing theoretical support and new ideas for the treatment of psoriasis with TCM medicine.
