Effect of Abnormal C Reactive Protein and Glycosylated Hemoglobin on the Occurrence of Major Adverse Cardiovascular Events in Patients with Unstable Angina

摘要 目的:探讨C反应蛋白(CRP)和糖化血红蛋白(HbA1c)异常对不稳定型心绞痛(UA)患者主要不良心血管事件(MACE)发生的影响。方法:回顾性收集2019年1月—2021年9月在萍乡市人民医院心内科收治的120例UA患者临床资料,保持随访1年。依据随访期内是否发生MACE分为发生组(n=21)与未发生组(n=99)。比较两组生命体征、超声心动图指标、实验室指标等基线资料。分析CRP、HbA1c对UA患者发生MACE的预测价值。结果:两组年龄、性别、心率(HR)、收缩压(SBP)、舒张压(DBP)、体重指数(BMI)、左心室舒张末期内径(LVEDD)、左心室射血分数(LVEF)、左心房内径(LAD)、总胆固醇(TC)及高密度脂蛋白(HDL)比较,差异均无统计学意义(P>0.05)。发生组CRP、HbA1c均高于未发生组,差异均有统计学意义(P<0.05)。多因素logistic回归分析,结果显示CRP、HbA1c升高是UA患者发生MACE的危险因素(P<0.05)。受试者操作特征(ROC)曲线显示CRP、HbA1c对UA患者发生MACE预测价值的结果显示曲线下面积分别为0.981、0.924。结论:CRP、HbA1c水平异常的UA患者具有更高的MACE风险,且CRP、HbA1c水平能有效预测MACE的发生。因此,临床应警惕UA患者CRP、HbA1c水平变化。 Objective:To investigate the effect of abnormal C reactive protein(CRP)and glycosylated hemoglobin(HbA1c)on the occurrence of major adverse cardiovascular events(MACE)in patients with unstable angina(UA).Method:The clinical data of 120 patients with UA admitted to the Department of Cardiology,Pingxiang People's Hospital from January 2019 to September 2021 were collected retrospectively,and followed up was maintained for one year.According to whether MACE occurred during the follow-up period,the patients were divided into occurrence group(n=21)and non-occurrence group(n=99).The baseline data such as vital signs,echocardiographic indicators and laboratory indicators of the two groups were compared.The predictive value of CRP and HbA1c for MACE in patients with UA was analyzed.Result:There were no statistically significant differences in age,gender,heart rate(HR),systolic blood pressure(SBP),diastolic blood pressure(DBP),body mass index(BMI),left ventricular end diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),left atrial diameter(LAD),total cholesterol(TC)and high-density lipoprotein(HDL)between the two groups(P>0.05).CRP and HbA1c in the occurrence group were higher than those in the non-occurrence group,the differences were statistically significant(P<0.05).Multivariate logistic regression analysis found that elevated CRP and HbA1c were risk factors for MACE in patients with UA(P<0.05).The receiver operating characteristic(ROC)curve analysis showed that the areas under the curve of CRP and HbA1c for predicting MACE in patients with UA were 0.981 and 0.924.Conclusion:Patients with UA and abnormal levels of CRP and HbA1c have a higher risk of MACE,CRP and HbA1c levels can help to predict the occurrence of MACE.Therefore,attention should be paid to the changes of CRP and HbA1c levels in patients with UA in clinical practice.
