急性心肌梗死患者血浆miR-195、sST2、Myo、CatS的检测价值

摘要 目的分析急性心肌梗死(AMI)患者血浆微小RNA-195(miR-195)、可溶性生长刺激表达因子2(sST2)、肌红蛋白(Myo)和组织蛋白酶S(CatS)的检测价值。方法选择2019年4月至2020年10月平阳县人民医院收治的AMI患者105例作为观察组,另择同期本院健康体检者94名作为对照组。检测并比较两组对象血浆miR-195、sST2、Myo和CatS水平。随访6个月,根据预后情况将患者分为预后不良组24例和预后良好组81例。对两组患者血浆miR-195、sST2、Myo和CatS水平进行比较,分析血浆miR-195、sST2、Myo和CatS对AMI患者预后的预测价值。结果观察组患者血浆miR-195、sST2、Myo和CatS水平高于对照组,差异均有统计学意义(均P<0.05)。经Pearson相关分析显示,血浆miR-195、sST2、Myo和CatS之间均呈正相关(均P<0.05)。预后不良亚组患者血浆miR-195、sST2、Myo和CatS水平高于预后良好亚组,差异均有统计学意义(均P<0.05)。单因素及多因素Cox回归分析显示,血浆miR-195、sST2、Myo和CatS水平异常升高均是导致AMI患者预后不良的危险因素(均P<0.05)。血浆miR-195、sST2、Myo和CatS 4者联合判断AMI患者预后的AUC值、灵敏度均高于单一检测(均P<0.05)。结论血浆miR-195、sST2、Myo和CatS的高表达与AMI患者预后不良相关。 Objective To investigate the detection value of plasma microRNA-195(miR-195),soluble growth stimulation expressed gene 2(sST2),myoglobin(Myo)and cathepsin S(CatS)in patients with acute myocardial infarction(AMI).Methods One hundred and five patients with AMI(observation group)and 94 healthy individuals(control group)were enrolled in this study between April 2019 and October 2020 in Pingyang County People's Hospital.Plasma miR-195,sST2,Myo and CatS were measured.All patients were followed up for 6 months and then were divided into poor outcome group(24 cases)and good outcome group(81 cases)based on the disease prognosis.Plasma miR-195,sST2,Myo and CatS were compared between the two groups.The predictive value of those parameters for the prognosis of AMI patients was analyzed.Results Plasma miR-195,sST2,Myo and CatS were significantly higher in the observation group than in the control group(all P<0.05).Pearson correlation analysis showed a positive correlation between plasma miR-195,sST2,Myo,and CatS(all P<0.05).Plasma miR-195,sST2,Myo,and CatS were significantly higher in poor outcome group than in good outcome group(all P<0.05).Univariate analysis and multivariate Cox regression analysis revealed that abnormally elevated plasma miR-195,sST2,Myo and CatS were the risk factors for poor prognosis of AMI patients(all P<0.05).The AUC value and sensitivity of a combination of miR-195,sST2,Myo and CatS for predicting prognosis of AMI patients were significantly higher than those of each index(all P<0.05).Conclusion High expression of plasma miR-195,sST2,Myo and CatS is related to poor prognosis of AMI patients.
