摘要
目的:精神科实习对结构式精神访谈的要求较高,本研究引入增强版卡尔加里-剑桥指南(Calgary-Cambridge guides, C-CG)教学,并探讨其在精神科实习生临床医患沟通中的实用性。方法:临床专业实习医生精神科实习阶段,选取在西安交通大学第一附属医院实习的50名实习医生接受增强版C-CG教学培训作为训练组;50名实习生接受传统临床沟通教学作为常规组,两组在入科及出科应用迷你版临床演练评估表(Mini-CEX)评估使用效果。结果:常规组和训练组实习医生入科第1周末Mini-CEX评分各项指标及师生满意度均未发现明显异常(P均>0.05),第3周末Mini-CEX评分组间比较结果显示训练组各项评分及师生满意度均高于常规组(P均<0.001),对第1周末和第3周末的Mini-CEX评分结果进行组内比较发现,常规组和训练组得分均有明显提升(P均<0.001)。结论:增强版C-CG教学培训较传统教学能更好地提高实习医生的临床沟通技能,临床上进行医患沟通技能培训有必要且对实习技能有提升作用。
Objective:Psychiatric practice had high requirements for structured psychiatric interviews.This study introduced the enhanced Calgary Cambridge guides(C-CG)teaching,and discussed its practicality in clinical doctor-patient communication among psychiatric interns.Method:In the psychiatric practice stage of clinical interns,50 interns in First Affiliated Hospital of Medical College of Xi'an Jiaotong University were selected to receive the enhanced C-CG training as the training group;50 interns received traditional clinical communication teaching as the routine group,and the mini clinical evaluation excercise(CEX)scale was used to evaluate the effect of the two groups in enrollment and graduation.Results:In the first week of admission,the mini CEX scores of interns and the satisfaction of teachers and students in the routine group and the training group showed no significant difference(all P>0.05).In the third week,the mini CEX scores and the satisfaction of teachers and students in the training group were higher than those in the routine group(all P<0.001).By comparing the mini CEX scores in the first week and the third week,the scores of the routine group and the training group were significantly improved(all P<0.001).Conclusion:The enhanced C-CG training could better improve the clinical communication skills of interns than the traditional teaching.It was necessary to carry out the training of doctor-patient communication skills in clinical practice,and it could improve the internship skills.
