摘要
目的探讨危害分析和关键控制点法(HACCP)在住院患者自备口服药管理中的应用效果。方法选取2020年12月至2021年5月乌鲁木齐市妇幼保健院(以下简称“本院”)的184名护士和208例患者作为研究对象,按照HACCP改进措施实施前后分为观察组和对照组,观察组包括96名护士、106例患者,对照组包括88名护士、102例患者。比较实施前后住院患者口服自备药差错发生情况及护士和患者的满意度情况。结果本院建立HACCP,绘制住院患者口服用药管理流程图,根据HACCP原则制订计划[包括危害分析,确定关键控制点(CCP),建立关键限值、监测项目、改进措施、记录],确定3个CCP,即出院带药、自备药物管理、自备药物医嘱。针对上述CCP制订并实施相应的管控措施,实现对自备药物差错风险的有效管控。两组自备口服药物差错发生率比较,差异有统计学意义(P<0.05)。结论将HACCP科学地应用于医院患者自备药环节的风险管理中,可以有效控制自备药物使用的差错风险,保证住院患者的用药安全。
Objective To explore the effect of hazard analysis and critical control point(HACCP)in the management of self-prepared oral medicine for inpatients.Methods A total of 184 nurses and 208 patients from Urumqi Maternal and Child Health Care Hospital(hereinafter referred to as"our hospital")from December 2020 to May 2021 were selected as the research objects,and they were divided into observation group and control group according to the implementation of HACCP improvement measures.The observation group included 96 nurses and 106 patients.The control group included 88 nurses and 102 patients.The occurrence of errors of self-prepared oral medicine in hospitalized patients and the satisfaction of nurses and patients were compared before and after implementation.Results HACCP was established in our hospital,the flow chart of oral medicine administration for inpatients was drawn,and plans were made according to HACCP principles(including hazard analysis,identification of critical control point[CCP],establishment of critical limits,monitoring items,improvement measures and records),and 3 CCPs were identified,namely discharge medication,self-prepared medication management and self-prepared medication orders.Based on the above CCP,corresponding control measures have been formulated and implemented to effectively control the risk of self-prepared medicine errors.The difference was statistically significant in the incidence of self-prepared oral medication error between two groups(P<0.05).Conclusion The scientific application of HACCP in the risk management of self-prepared medicine can effectively control the error risk of self-prepared medicine and ensure the medicine safety of inpatients.
