摘要
目的:探讨诊断性取血与非危重症内科住院患者血红蛋白变化的关系。方法:连续回顾非危重症内科住院患者,记录年龄、性别、住院时间、入院时血红蛋白、出院时血红蛋白、总采血管数、平均采血管数、血红蛋白变化、心血管疾病、呼吸系统疾病和贫血等。应用多因素线性回归以及曲线拟合来检测诊断性取血量与血红蛋白变化的关系。结果:82例患者平均年龄(68.9±11.4)岁,52.4%为男性。单因素线性回归(β=0.39,95%CI:-0.09~0.86,P=0.113)和多因素线性回归(β=0.29,95%CI:-0.20~0.79,P=0.248)显示总采血管数与血红蛋白变化值无关。然而曲线拟合显示二者存在非线性关系(P=0.022)。计算拐点为12,拐点两侧β值和置信区间分别为-2.02(95%CI:-4.10~0.06,P=0.065)和0.89(95%CI:0.25~1.53,P=0.010)。结论:在非危重症内科住院患者中,诊断性取血与血红蛋白变化之间的关系是非线性的。需要进一步的研究来阐明血红蛋白变化对这些患者的长期影响。
Objective:To investigate the relationship between diagnostic blood draws and hemoglobin changes in non-critically ill hospitalized inpatients.Methods:Consecutive non-critically ill hospitalized patients in medicine ward were reviewed.Clinical data such as age,sex,length of stay,admission hemoglobin,discharge hemoglobin,total tubes,average tubes,change of hemoglobin,cardiovascular disease,respiratory disease,and discharge anemia were recorded.Multifactorial linear regression as well as curve fitting were applied to test the relationship between diagnostic blood draws and hemoglobin changes.Results:The average age of 82 participants was(68.9±11.4)years old,and about 52.4%was male.Result of fully-adjusted linear regression showed diagnostic blood draws was not associated with change of hemoglobin in univariate(β=0.39,95%CI:-0.09~0.86,P=0.113)and multivariate linear regression analysis(β=0.29,95%CI:-0.20~0.79,P=0.248).However,Non-linear relationship was detected when the smooth curve fitting was conducted(P=0.022).With the inflection point of 12,the estimatedβand confidence intervals of the left and right sides of inflection point were-2.02(95%CI:-4.10~0.06,P=0.065)and 0.89(95%CI:0.25~1.53,P=0.01),respectively.Conclusion:The relationship detected between total tubes of diagnostic blood draws and decreasing of hemoglobin was nonlinear in non-critically ill inpatients.Further study was needed to clarify the long-term effects of the hemoglobin decreasing in these patients.
