地舒单抗在大鼠脆性骨折愈合中的疗效观察

摘要 目的:研究地舒单抗在大鼠骨质疏松脆性骨折愈合过程中的影响。方法:选取6月龄SD雌性大鼠60只,按随机数字表法分为三组A组(股骨骨折)、B组(去卵巢+股骨骨折+地舒单抗治疗组)、C组(去卵巢+股骨骨折模型组),每组20只。B、C两组大鼠行去势手术切除卵巢建立骨质疏松模型,三组大鼠分别行股骨骨折克氏针内固定,对比分析各组大鼠骨折术后3、6周的愈合情况,双能X线骨密度吸收仪检测股骨骨密度(BMD),生物力学检测仪测量股骨最大载荷值、最大载荷回复率变化,DR摄影和HE染色观察影像学以及组织显微形态学差异。结果:骨折术后第3周时,B、C两组大鼠股骨骨密度相当,差异无统计学意义(P>0.05),两组骨密度含量均低于A组大鼠(P<0.05)。骨折术后第6周时,三组大鼠股骨骨密度均升高,表现为A组>B组>C组,差异有统计学意义(P<0.05)。在骨折术后第6周时各组大鼠间生物力学测量值(最大载荷值、最大载荷回复率)较第3周时均有明显增加,但仍表现为A组>B组>C组,各组间差异均有统计学意义(P<0.05)。影像及病理切片观察结果显示,骨折愈合过程中B组较C组骨膜反应更深,骨痂体积更厚实,骨小梁更密集,骨细胞密度更高。结论:骨质疏松骨折术后应用地舒单抗不干扰骨痂的形成,具有更快提升患处骨密度,增加骨强度,促进骨折愈合的作用。 Objective:Study on the efficacy of denosumab in the healing of fragility fractures in rats.Methods:60 healthy 6-month-old female SD rats were divided into three groups randomly with 20 rats in each group:group A(femur fracture),group B(ovariectomized+femur fracture+denosumab treatment group),group C(ovariectomized+femur fracture group).Rats in B and C groups were ovariectomized to establish osteoporosis model,all three groups of rats underwent internal fixation of femoral fractures.Compare the changes of rats in each group at 3 and 6 weeks after fracture operation,including bone mineral density,maximum femoral load value,maximum load recovery rate,variations on imaging and histology.Results:At the 3rd week after the fracture operation,the femoral bone density of rats in groups B and C were similar,but the values of these two groups were lower than group A(P<0.05).At the 6th week after the fracture operation,the femoral bone density of rats in the three groups was increased and ranked up-down in order of group A>group B>group C,the difference was statistically significant(P<0.05).At the 6th week,the biomechanical measurement values(maximum load value and maximum load recovery rate)of the rats in each group were significantly increased compared with those at the 3rd week,and it also expressed as group A>group B>group C.Observation of imaging and pathological sections showed that during the fracture healing process,group B had deeper periosteal reaction,thicker callus,denser bone trabeculae,and higher density of bone cells than group C.Conclusion:For osteoporotic fractures,denosumab does not interfere with callus formation,and helps to increase bone density and bone strength more effectively,which is conducive to fracture healing.
