摘要
目的:比较氨甲环酸两种不同给药途径在全膝关节置换术中的应用效果。方法:对2021年1—12月萍乡市人民医院收治的100例全膝关节置换术患者进行研究,随机分为治疗组(50例)和对照组(50例)。对照组采用关节腔灌注氨甲环酸,治疗组采用关节腔灌注氨甲环酸+关节囊周围注射氨甲环酸。比较两组出血量、美国特种外科医院(HSS)膝关节评分、血清C反应蛋白(CRP)、血红蛋白(Hb)、外周血白细胞计数(WBC)、红细胞沉降率(ESR)及皮下瘀斑发生情况。结果:治疗组失血量较对照组低,差异有统计学意义(P<0.05)。术后3、6周,治疗组HSS膝关节评分均高于对照组,差异均有统计学意义(P<0.05)。术后1、7 d,治疗组血清CRP、外周血WBC、ESR均低于对照组,治疗组Hb均高于对照组,差异均有统计学意义(P<0.05)。两组皮下瘀斑发生率差异无统计学意义(P>0.05)。结论:关节腔灌注联合关节囊周围注射氨甲环酸在全膝关节置换术应用中,可有效减少患者出血量,改善术后Hb水平,降低术后炎症反应,促进患者膝关节功能恢复。
Objective:To compare the application effects of two different administration routes of Tranexamic Acid in total knee arthroplasty.Method:A total of 100 patients with total knee arthroplasty who were admitted to Pingxiang People's Hospital from January to December 2021 were randomly divided into the treatment group(50 cases)and the control group(50 cases).The control group was treated with intra-articular infusion of Tranexamic Acid,and the treatment group was treated with intra-articular infusion of Tranexamic Acid and intra-articular injection of Tranexamic Acid around the joint capsule.The blood loss,American hospital for special surgery(HSS)knee score,serum C reactive protein(CRP),hemoglobin(Hb),peripheral blood white blood cell count(WBC),erythrocyte sedimentation rate(ESR)and the occurrence of subcutaneous ecchymosis were compared between the two groups.Result:The blood loss in the treatment group was lower than that in the control group(P<0.05).At 3 and 6 weeks after operation,the HSS scores of the treatment group were higher than those of the control group,the differences were statistically significant(P<0.05).At 1 and 7 d after operation,the serum CRP,peripheral blood WBC and ESR in the treatment group were lower than those in the control group,and the Hb were higher than those in the control group,the differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of subcutaneous ecchymosis between the two groups(P>0.05).Conclusion:Intra-articular infusion combined with intra-articular injection around the joint capsule of Tranexamic Acid in total knee arthroplasty can effectively reduce the blood loss,improve the postoperative Hb level,lower the postoperative inflammatory response,and promote the recovery of knee joint function.
