摘要
目的在肛周封闭治疗基础上,观察耳穴贴压联合穴位埋线治疗慢性肛周湿疹的临床疗效和安全性。方法将80例慢性肛周湿疹患者按随机数字表法分为观察组(40例,脱落1例)和对照组(40例,脱落2例)。两组患者均采用亚甲蓝混合液肛周封闭治疗,观察组另予耳穴贴压联合穴位埋线治疗,对照组另予单纯耳穴贴压治疗。观察两组治疗前后肛周瘙痒程度评分、肛周潮湿程度评分、肛周皮损面积评分、肛周皮损形态评分、皮肤病生活质量指数(dermatology life quality index,DLQI)评分的变化,比较两组临床疗效、不良反应发生情况及复发情况。结果治疗后,两组肛周瘙痒程度评分、肛周潮湿程度评分、肛周皮损面积评分、肛周皮损形态评分、DLQI评分均较治疗前降低(P<0.05),且观察组上述各项评分均低于对照组(P<0.05)。观察组总有效率高于对照组(P<0.05)。观察组复发率低于对照组(P<0.05)。治疗过程中两组均未发生严重不良反应。结论在肛周封闭治疗基础上,耳穴贴压联合穴位埋线治疗慢性肛周湿疹可有效缓解肛周瘙痒和潮湿的症状,改善肛周局部皮损形态,提高患者生活质量,临床疗效优于单一耳穴贴压,且复发率较低。
Objective To observe the clinical efficacy and safety of auricular point sticking plus catgut embedding in treating chronic perianal eczema based on perianal block therapy.Method Eighty patients with chronic perianal eczema were divided into an observation group(40 cases,1 dropout)and a control group(40 cases,2 dropouts)using the random number table method.Perianal block therapy with methylene blue mixture was offered to both groups.In addition,the observation group received auricular point sticking plus catgut embedding,and the control group received auricular point sticking.For both groups,the perianal itching intensity score,perianal damp intensity score,perianal skin lesion size and morphology scores,and dermatology life quality index(DLQI)score were observed before and after the treatment.The clinical efficacy,adverse reactions,and recurrence were compared between the two groups.Result After the treatment,the perianal itching intensity score,perianal damp intensity score,perianal skin lesion size and morphology scores,and DLQI score dropped in both groups(P<0.05)and were lower in the observation group than in the control group(P<0.05).The observation group had a higher total effective rate than the control group(P<0.05),and the relapse rate was lower in the observation group than in the control group(P<0.05).No significant adverse reactions happened during the treatment process in either group.Conclusion Based on perianal block therapy,auricular point sticking plus catgut embedding can relieve perianal itching and dampness,improve perianal skin lesion features,and enhance the quality of life in treating chronic perianal eczema,producing more significant efficacy than the sole use of auricular point sticking with a lower relapse rate.
