摘要
目的:分析腰麻-硬膜外联合麻醉在吻合器痔上黏膜环切术中的麻醉效果。方法:选取2020年5月—2021年6月于汕头市潮南区人民医院行吻合器痔上黏膜环切术的患者108例作为研究对象,根据随机数字表法分为两组,各54例。对照组行常规肛周神经麻醉,观察组行腰麻-硬膜外联合麻醉。比较两组疼痛情况、麻醉效果、并发症发生情况、手术满意度。结果:术后1、2、4、8、15 h,观察组疼痛评分低于对照组,差异有统计学意义(P<0.001)。观察组麻醉起效时间短于对照组,肌肉松弛效果优秀率高于对照组,差异有统计学意义(P<0.05)。观察组不良反应总发生率低于对照组,差异有统计学意义(P=0.002)。观察组手术总满意度高于对照组,差异有统计学意义(P<0.05)。结论:腰麻-硬膜外联合麻醉在吻合器痔上黏膜环切术中的麻醉效果较好,能够减轻患者术后疼痛程度,缩短麻醉起效时间,改善肌肉松弛效果,减少不良反应,提高手术满意度。
Objective:To analyze the anesthesia effect of combined spinal-epidural anesthesia in stapled hemorrhoid mucosal circumcision.Methods:A total of 108 patients who underwent stapled hemorrhoid mucosal circumcision in Shantou Chaonan District People′s Hospital from May 2020 to June 2021 were selected as the study subjects.They were divided into two groups by random number method,with 54 patients in each group.The control group underwent conventional perianal nerve anesthesia,and the observation group underwent combined spinal-epidural anesthesia.The pain,anesthesia effect,occurrence of complications and surgery satisfaction were compared between two groups.Results:At 1,2,4,8,and 15 h after surgery,the pain scores in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The onset time of anesthesia in the observation group was shorter than that in the control group,and the excellent rate of muscle relaxation effect in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.002).The total surgery satisfaction in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The combined spinal-epidural anesthesia has better anesthesia effects in stapled hemorrhoid mucosal circumcision and can relieve postoperative pain intensity of patients,shorten the onset time of anesthesia,improve the muscle relaxation effect,reduce adverse reactions and improve surgery satisfaction.
