剪切波弹性成像技术定量评估在早期糖尿病肾病中的临床意义

摘要 目的探析早期糖尿病肾病患者应用剪切波弹性成像(SWE)技术定量评估的应用价值。方法150例糖尿病肾病患者,根据尿白蛋白与肌酐的比值(ACR)水平分为A组(正常白蛋白尿,ACR<30 mg/g)、B组(微量白蛋白尿,ACR在30~300 mg/g)、C组(临床蛋白尿,ACR>300 mg/g),各50例;另选取50例健康体检无糖尿病肾病者作为对照组。所有研究对象均进行SWE检测。比较四组肾实质弹性模量数值[最大值(Emax)、平均值(Emean)以及最小值(Emin)]及CysC水平,分析早期糖尿病肾病弹性模量数值与CysC的相关性。结果对照组Emax、Emean、Emin分别为(5.50±0.57)、(3.12±0.47)、(1.62±0.32)kPa,A组Emax、Emean、Emin分别为(6.38±1.10)、(3.77±0.64)、(2.00±0.50)kPa,B组Emax、Emean、Emin分别为(7.05±0.68)、(4.42±0.77)、(2.48±0.54)kPa,C组Emax、Emean、Emin分别为(9.77±1.10)、(6.30±0.98)、(3.43±0.54)kPa。A组、B组、C组Emax、Emean、Emin水平均高于对照组,B组、C组Emax、Emean、Emin水平均高于A组,C组Emax、Emean、Emin水平均高于B组,四组比较差异具有统计学意义(P<0.05)。对照组、A组、B组、C组的CysC分别为(0.82±0.12)、(0.92±0.12)、(1.62±0.43)、(2.22±0.51)mg/L。A组、B组、C组CysC水平均高于对照组,B组、C组CysC水平均高于A组,C组CysC水平均高于B组,四组比较差异具有统计学意义(P<0.05)。早期糖尿病肾病患者肾实质弹性模量数值Emax、Emean、Emin与CysC呈正相关(r=0.653、0.853、0.662,P<0.05)。结论早期糖尿病肾病患者应用SWE进行肾受损程度评价具有无创、定量、动态的优势,对于掌握患者病情并制定对应治疗方案有良好的指导价值。 Objective To investigate the practical value of shear wave elastography(SWE)in quantitative evaluation of early diabetes nephropathy.Methods A total of 150 diabetes nephropathy patients were selected and divided into group A[normal albuminuria,albumin-to-creatinine ratio(ACR)<30 mg/g],group B(microalbuminuria,ACR 30-300 mg/g)and group C(clinical albuminuria,ACR>300 mg/g),with 50 cases each,according to ACR.Another 50 healthy patients without diabetes nephropathy were selected as a control group.All subjects underwent SWE testing.The values of maximum,average and minimum elastic modulus of renal parenchyma[Emax,Emean and Emin]and CysC levels in the four groups were compared to analyze the correlation between elastic modulus and CysC in early diabetes nephropathy.Results In control group,Emax,Emean and Emin were(5.50±0.57),(3.12±0.47)and(1.62±0.32)kPa,respectively;in group A,Emax,Emean and Emin were(6.38±1.10),(3.77±0.64)and(2.00±0.50)kPa,respectively;in group B,Emax,Emean and Emin were(7.05±0.68),(4.42±0.77)and(2.48±0.54)kPa,respectively;in group C,Emax,Emean and Emin were(9.77±1.10),(6.30±0.98)and(3.43±0.54)kPa,respectively.Emax,Emean and Emin in group A,group B and group C were higher than those in the control group;Emax,Emean and Emin in group B and group C were higher than those in group A;Emax,Emean and Emin in group C were higher than those in group B;the differences among the four groups were statistically significant(P<0.05).CysC of control group,group A,group B and group C were(0.82±0.12),(0.92±0.12),(1.62±0.43)and(2.22±0.51)mg/L,respectively.CysC level in group A,group B and group C was higher than that of control group,CysC level in group B and group C was higher than that in group A,and CysC level in group C was higher than that in group B.The differences were statistically significant(P<0.05).Emax,Emean,Emin were positively correlated with CysC in early diabetes nephropathy patients(r=0.653,0.853,0.662;P<0.05).Conclusion SWE has the advantages of non-invasive,quantitative and dynamic i
