2018—2022年某医院儿科临床分离病原菌种类及耐药趋势分析

Analysis of clinical isolated pathogenic bacteria species and drug resistance trends in a hospital pediatric department from 2018—2022

摘要 目的回顾性分析住院患儿临床分离的病原菌种类和耐药性变迁,为临床合理选择使用抗菌药物提供指导依据。方法对2018年1月—2022年12月某医院收治患儿细菌学标本的来源、病原菌分离情况和药敏试验资料进行分析。结果2018—2022年共分离病原菌809株,其中革兰阴性菌476株(58.83%)、革兰阳性菌333株(41.16%)。分离的主要病原菌为大肠埃希菌208株(25.71%)、屎肠球菌96株(11.87%)、肺炎克雷伯菌51株(6.30%)、金黄色葡萄球菌43株(5.32%)和鲍曼不动杆菌43株(5.32%)。主要标本来源为中段尿标本366株(45.24%)、痰液标本168株(20.77%)、血液标本121株(14.96%)。大肠埃希菌对亚胺培南、哌拉西林;他唑巴坦和头孢他啶敏感,对氨苄西林和左旋氧氟沙星耐药率呈上升趋势(P<0.05)。金黄色葡萄球菌对万古霉素、替加环素和利奈挫胺均敏感,对青霉素耐药率均>90%。共检出多重耐药菌24株,耐甲氧西林金黄色葡萄球菌检出率为30.23%,多重耐药鲍曼不动杆菌检出率为20.93%。结论该医院儿科临床分离的革兰阴性菌检出率逐年增多,对三代头孢类抗菌药物保持高耐药性、对喹诺酮类抗菌药物耐药率逐渐增高。 Objective To retrospectively analyze the types of clinically isolated pathogenic bacteria and changes in drug resistance in hospitalized children,and to provide a guiding basis for the rational clinical selection of antimicrobial agents.Methods The sample sources,pathogenic bacteria isolation and drug sensitivity test information of bacteriological specimens of children admitted to a hospital from January 2018 to December 2022 were analyzed.Results A total of 809 pathogenic bacteria were isolated from 2018—2022,including 476(58.83%)Gram-negative and 333(41.16%)Gram-positive bacteria.The main pathogenic bacteria isolated were Escherichia coli 208 strains(25.71%),Enterococcus faecalis 96 strains(11.87%),Klebsiella pneumoniae 51 strains(6.30%),Staphylococcus aureus 43 strains(5.32%)and Acinetobacter baumannii 43 strains(5.32%).The main sources of specimens were 366(45.24%)middle urine specimens,168(20.77%)sputum specimens,and 121(14.96%)blood specimens.There was an increasing trend of Escherichia coli resistance to ampicillin and levofloxacin(P<0.05),and sensitivity to imipenem,piperacillin;tazobactam and ceftazidime.Staphylococcus aureus was sensitive to vancomycin,tigecycline and linaclotide,and the resistance rate to penicillin was>90%.A total of 24 multi-drug resistant strains were detected,and the detection rate of methicillin-resistant S.aureus was 30.23%(13;43 strains),while the detection rate of multi-drug resistant Acinetobacter baumannii was 20.93%(9;43 strains).Conclusion The detection rate of Gram-negative bacteria isolated from the pediatric clinic of this hospital has been increasing year by year,maintaining high resistance to three generations of cephalosporins and gradually increasing the resistance rate to quinolones.
