摘要
目的:探讨术前手部按摩联合贝克认知疗法在老年性白内障手术患者中的应用效果。方法:选取2021年6月—2022年6月中国人民解放军联勤保障部队第九一〇医院收治的100例老年性白内障手术患者作为研究对象。根据随机数表法将其分为对照组(n=50)和观察组(n=50)。对照组术前给予手部按摩,观察组术前在对照组基础上给予贝克认知疗法。比较两组入组时、麻醉后生理指标、应激指标,干预前后焦虑情况。结果:麻醉后,两组心率(heart rate,HR)、舒张压(diastolic blood pressure,DBP)和收缩压(systolic blood pressure,SBP)均升高,观察组HR、DBP、SBP均低于对照组,差异有统计学意义(P<0.05)。麻醉后,两组皮质醇(cortisol,Cor)、促肾上腺皮质激素(adrenocorticotropic hormone,ACTH)水平均升高,观察组Cor、ACTH水平均低于对照组,差异有统计学意义(P<0.05)。干预后,两组状态焦虑(STAI-S)评分、特质焦虑(STAI-T)评分均降低,观察组STAI-S评分、STAI-T评分均低于对照组,差异有统计学意义(P<0.05)。结论:术前手部按摩联合贝克认知疗法应用于老年性白内障手术患者中可以改善手术应激情况和心理状态。
Objective:To explore the application effect of preoperative hand massage combined with Beck's cognitive therapy in senile cataract patients undergoing surgery.Method:A total of 100 patients with senile cataract undergoing surgery admitted to the 910th Hospital of the Chinese People's Liberation Army Joint Logistics Support Force from June 2021 to June 2022 were selected as the study objects.They were divided into control group(n=50)and observation group(n=50)according to random number table method.The control group received hand massage before surgery,and the observation group received Beck's cognitive therapy on the basis of the control group before surgery.The physiological indexes,stress indexes at enrollment and after anesthesia and anxiety condition before and after intervention were compared between the two group.Result:After anesthesia,the heart rate(HR),diastolic blood pressure(DBP)and systolic blood pressure(SBP)in both groups were increased,and HR,DBP and SBP in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).After anesthesia,the levels of cortisol(Cor)and adrenocorticotropic hormone(ACTH)in both groups were increased,and the levels of Cor and ACTH in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).After intervention,the scores of state anxiety(STAI-S)and trait anxiety(STAI-T)were decreased in both groups,and the scores of STAI-S and STAI-T in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The application of preoperative hand massage combined with Beck's cognitive therapy can improve surgical stress condition and psychological status in patients with senile cataract undergoing surgery.
