摘要
目的:探究导管“O”型环绕二次固定法在普外科术后切口周围引流管固定中的应用效果。方法:前瞻性选取2022年7月—2023年1月阳江市人民医院普外科术后留置引流管患者160例作为研究对象,按随机数表法将其分为常规组与改良组,各80例。常规组采用高举平台二次固定法,改良组采用导管“O”型环绕二次固定法。比较固定两组的引流固定效果(导管折叠受压发生率、皮肤切口因牵拉出血发生率、非计划拔管发生率)、导管累计再次固定次数、更换固定材料间隔天数及平均住院时间;采用舒适状况量表(GCQ)评价两组舒适性;自制问卷调查两组满意度。结果:改良组导管折叠受压发生率、皮肤切口因牵拉出血发生率、非计划拔管发生率均低于常规组,差异有统计学意义(P<0.05)。改良组导管累计再次固定次数少于常规组,住院时间短于常规组,更换固定材料间隔天数长于常规组,差异有统计学意义(P<0.05)。改良组生理、心理精神、社会文化、环境及GCQ总分,满意度得分均高于常规组,差异有统计学意义(P<0.05)。结论:导管“O”型环绕二次固定法在普外科术后切口周围引流管固定中应用效果显著,能够有效固定导管,且避免导管直接接触摩擦皮肤,有助于提升患者舒适度,提高患者满意度,减少非计划拔管发生,确保留置引流管患者的安全,缩短住院时间。
Objective:Application effect of catheter"O"shaped encircle secondary fixation method in fixation drainage tube around incision after general surgery.Method:A total of 160 patients fixation drainage tube after general surgery in the General Surgery Department of Yangjiang People's Hospital from July 2022 to January 2023 were prospectively selected as the study objects,and they were divided into conventional group and improved group according to random number table method,with 80 patients in each group.The conventional group adopts the method of secondary fixation of the raised platform,and the improved group adopts catheter"O"shaped encircle secondary fixation method.The effects of drainage fixation(incidence of catheter folding compression,skin incision bleeding due to traction,unplanned extubation),cumulative refixation times of the catheter,days between replacement of fixed material and average length of hospital stay were compared between the two groups.General comfort questionnaire(GCQ)was used to evaluate the comfort of the two groups.Self-made questionnaire to investigate the satisfaction of the two groups.Result:The incidence rates of catheter folding compression,skin incision bleeding due to traction,unplanned extubation in the improved group were lower than those in the conventional group,the differences were statistically significant(P<0.05).The cumulative refixation times of catheter in the improved group was less than that in the conventional group,the hospital stay was shorter than that in the conventional group,and the days between replacement of fixed materials were longer than that in the conventional group,the differences were statistically significant(P<0.05).The physiological,psychological,sociocultural,environmental,GCQ total score and satisfaction scores of the improved group were higher than those of the conventional group,and the differences were statistically significant(P<0.05).Conclusion:Catheter"O"shaped encircle secondary fixation method has significant application effect in the fixati
