摘要
目的基于单中心资料的回顾性分析,探讨胸部LDCT筛查的体检人群中冠状动脉钙化(CAC)检出状况及有关的危险因素。方法收集2020—2021年首次在华东疗养院行胸部LDCT筛查的40岁以上符合条件的22500例体检者,年龄40~90岁。搜集相关临床资料及血液生化检查结果,包括性别、年龄、既往史等19项可疑危险因素。对不同人群CAC检出率差异采用χ^(2)检验,CAC的独立危险因素进行二元Logistic回归分析。结果发现3306例患有CAC,检出率约为14.7%,男性显著高于女性,其中男性2640例(19.30%),女性666例(7.55%)。随着年龄的增加,CAC的检出率呈增加趋势。Logistic回归分析显示,性别(OR=1.850,95%CI:1.612,2.123)、年龄(OR=1.137,95%CI:1.131,1.144)、体质指数(BMI)(OR=1.027,95%CI:1.012,1.043)、家族慢性病史(OR=0.824,95%CI:0.748,0.908)、家族肿瘤史(OR=1.231,95%CI:1.110,1.366)、传染病史(OR=1.210,95%CI:1.082,1.352)、个人肿瘤史(OR=1.180,95%CI:1.038,1.342)、吸烟(OR=0.649,95%CI:0.586,0.718)、饮酒(OR=0.828,95%CI:0.744,0.921)、体育运动(OR=0.904,95%CI:0.828,0.988)、高脂血症(OR=0.897,95%CI:0.802,1.003)、高血压(OR=0.596,95%CI:0.544,0.653)、糖尿病(OR=0.619,95%CI:0.533,0.693)、脂肪肝(OR=1.030,95%CI:0.933,1.136)及Hp感染(OR=0.888,95%CI:0.796,0.991)与CAC发病相关;与手术外伤史、药物过敏史、高尿酸血症、空腹血糖受损等无相关性。结论40岁以上体检人群中CAC的检出率较高。该体检人群中影响CAC的独立危险因素有性别、年龄、BMI、家族慢性病史、家族肿瘤史、传染病史、个人肿瘤史、吸烟、饮酒、体育运动、高脂血症、高血压、糖尿病、脂肪肝及Hp感染等。针对危险因素提供相应的健康教育,预防冠心病的发生。
Objective To explore the detection rate of coronary artery calcification(CAC)by the chest low-dose computed tomography(LDCT)and risk factors based on a retrospective analysis of single-center data.Methods Clinical data and biochemical testing data of 22500 eligible individuals aged 40 to 90 years who were initially screened by LDCT in Huadong Sanatorium from 2020 to 2021 were collected,including 19 suspected risk factors like the gender,age,and past history.The detection rate of CAC among different populations was compared by the Chi-square test.Binary Logistic regression analysis was conducted to identify the independent risk factors for CAC.Results CAC was detected in 3306(14.7%),involving 2640 males(19.30%)and 666 females(7.55%).The detection rate of CAC was significantly higher in males than that of females,which increased with age.Multivariate Logistic regression analysis showed that gender(OR=1.850,95%CI:1.612,2.123),age(OR=1.137,95%CI:1.131,1.144),body mass index(BMI,OR=1.027,95%CI:1.012,1.043),family history of chronic diseases(OR=0.824,95%CI:0.748,0.908),family history of tumors(OR=1.231,95%CI:1.110,1.366),history of infectious diseases(OR=1.210,95%CI:1.802,1.352),personal history of tumors(OR=1.180,95%CI:1.038,1.342),smoking(OR=0.649,95%CI:0.586,0.718),alcohol consumption(OR=0.828,95%CI:0.744,0.921),physical exercise(OR=0.904,95%CI:0.828,0.988),hyperlipidemia(OR=0.897,95%CI:0.802,1.003),hypertension(OR=0.596,95%CI:0.544,0.653),diabetes(OR=0.619,95%CI:0.533,0.693),fatty liver(OR=1.030,95%CI:0.933,1.136),and Helicobacter Pylori(H.pylori)infection(OR=0.888,95%CI:0.796,0.991)were significantly correlated with the onset of CAC.It was not correlated with history of surgical trauma,drug allergy,hyperuricemia,and impaired fasting blood glucose.Conclusion The detection rate of CAC is high in the population aged 40 years and above.Gender,age,BMI,family history of chronic diseases,family history of tumors,infectious disease history,personal tumor history,smoking,drinking,sports,hyperlipidemia,fatty liver,hypertens
