摘要
目的探讨磁共振弥散加权成像(DWI)在脑转移瘤鉴别诊断中的准确率。方法选取河南省济源市第二人民医院2020年1月至2022年12月接收的80例脑转移瘤患者,设为观察组,另选取本院同期接收的脑胶质瘤患者80例,设为对照组,均予以磁共振平扫、DWI检查,分析DWI鉴别诊断脑转移瘤的准确性与不同区域表观弥散系数(ADC)值,并比较2组病灶及影像学表现。结果DWI在脑转移瘤鉴别诊断中的准确率是98%,敏感度是99%,特异度是83%;脑转移瘤肿瘤实质区、囊变区、边缘区、周围水肿区的ADC值均高于肿瘤对侧正常脑白质区(肿瘤实质区与肿瘤对侧正常脑白质区t=26.833,肿瘤囊变区与肿瘤对侧正常脑白质区t=94.337,肿瘤边缘区与肿瘤对侧正常脑白质区t=57.883,肿瘤周围水肿区与肿瘤对侧正常脑白质区t=68.528,P均<0.05);2组病灶形态、病灶周围水肿情况比较差异有统计学意义(χ^(2)=42.776、42.524,P均<0.05),病灶边缘情况比较则差异无统计学意义(P>0.05),观察组DWI等信号ADC值接近对侧率、DWI低信号ADC值较对侧升高率均高于对照组(χ^(2)=25.664、7.118,P均<0.05),DWI高信号ADC值较对侧升高率、DWI混杂信号ADC值较对侧升高率则低于对照组(χ^(2)=25.507、36.923,P均<0.05)。结论在脑转移瘤鉴别诊断中,DWI可进一步提升诊断准确率,其影像学表现与非脑转移瘤差异有统计学意义,可为鉴别诊断提供可靠依据。
Objective To investigate the accuracy of magnetic resonance imaging(MRI)diffusion weighted imaging(DWI)in the differential diagnosis of brain metastases.Methods A total of 80 patients with brain metastases who were admitted to Jiyuan Second People′s Hospital from January 2020 to December 2022 were selected as the observation group,and another 80 patients with brain glioma who were admitted to our hospital during the same period were selected as the control group.All patients were subjected to magnetic resonance plain scan and diffusion weighted imaging examination.The accuracy of MRI DWI in differential diagnosis of brain metastases and the apparent diffusion coefficient(ADC)values in different areas were analyzed,and the lesions and imaging performances of the two groups were compared.Results The accuracy,sensitivity and specificity of MRI DWI in the differential diagnosis of brain metastases were 97.50%,98.65%and 83.33%.The ADC values of the parenchymatous,cystic,marginal and peripheral edema areas of brain metastases were higher than those in the normal cerebral white matter area on the opposite side of the tumor(Tumor parenchyma vs tumor-contralateral normal white matter area t=26.833,tumor cystic lesion vs tumor-contralateral normal white matter area t=94.337,tumor margin vs tumor-contralateral normal white matter area t=57.883,and peritumor edema vs tumor-contralateral normal white matter area t=68.528,all P<0.05).There were significant differences in lesion morphology and peripheral edema between the two groups(χ^(2)=42.776、42.524,all P<0.05),while there was no significant difference in lesion margin(P>0.05),the ADC value of DWI signal was close to the opposite side rate,the ADC value of DWI low signal was higher than that of the control group(χ^(2)=25.664、7.118,all P<0.05),the rate of elevation of DWI high signal ADC value over the opposite side,and the DWI mixed signal ADC values over the contralateral side were lower than those of the control group(χ^(2)=25.507,36.923,all P<0.05).Conclusion In
