摘要
目的 探讨江苏地区胫骨平台骨折患者的流行病学特点及变化趋势。方法 选取2009-01-01-2018-12-31苏州大学附属张家港医院因胫骨平台骨折住院治疗的808例患者为调查对象,分析其临床资料,包括性别、年龄、骨折侧别、发病季节、致伤原因及骨折分型等。采用t检验和χ^(2)检验进行统计学分析。结果 808例胫骨平台骨折患者,男383例(47.40%),女425例(52.60%),男女比例为0.90∶1;左侧497例(61.51%),右侧311例(38.49%),左右比为1.60∶1。患者平均年龄为(51.98±13.85)岁,男性平均年龄为(48.59±13.70)岁,女性为(55.04±13.27)岁,女性大于男性,差异有统计学意义,t=6.797,P<0.001。患者发病高峰为40~69岁,其中男性发病高峰为40~49岁,女性为60~69岁。2009-2018年胫骨平台骨折发病人数整体呈升高趋势,其中2009年72例(8.91%),2018年最高101例(12.50%),各年份发病率比较差异有统计学意义,χ^(2)=5.444,P=0.020。胫骨平台骨折冬季发病率最高,为27.23%(220/808);夏季最低,为22.77%(184/808);但各个季节发病率相比差异无统计学意义,χ^(2)=4.924,P=0.177。交通事故伤是主要的致伤原因,占67.20%;其中电动自行车损伤所占比例最高,为48.88%。Schatzker分型中,Ⅱ型所占比例最高,为31.19%;其次为Ⅵ型,占19.06%。结论 胫骨平台骨折发病人数整体呈升高趋势,在性别、年龄、骨折侧别、发病季节、致伤原因及骨折分型等方面具有一定的流行病学分布特征,为进一步的制定防治策略提供理论依据。
Objective To investigate the epidemiological characteristics and annual trends of patients with tibial plateau fractures inJiangsu.Methods We retrieved clinical data of 808 patients with tibial plateau fractures admitted to Zhangjiagang Hospital Affiliated to Soochow University from January 1,2009 to December 31,2018.The gender,age,fracture side,admission season,injury mechanism and fracture classification were recorded,and t-test and Chi-squared test were used for statistical analysis.Results There were 808 patients with tibial plateau fractures,including 383 males(47.40%)and 425 females(52.60%),with a male to female ratio of 0.90:1.There were 497 cases on the left side(61.51%)and 311 cases on the right side(38.49%),with a left to right ratio of 1.60:1.The average age of onset of tibial plateau fractures was(51.98±13.85)years old,with male patients at(48.59±13.70)years old and female patients at(55.04±13.27)years old.The average age of females was higher than that of males,which was significantly different(t=6.797,P<0.001).The peak onset age of all patients was 40-69 years old,with a peak onset age of 40-49 years for males and 60-69 years for females.The overall incidence of tibial plateau fractures was on the rise from 2009 to 2018,with 72 cases(8.91%)in 2009 and a maximum of 101 cases(12.50%)in 2018.The incidence rate of each year was statistically significant(χ^(2)=5.444,P=0.020).The incidence of tibial plateau fractures was highest in winter at 27.23%(220/808)and it was lowest in summer at 22.77%(184/808).There was no significant difference in the distribution of population among different seasons(χ^(2)=4.924,P=0.177).Traffic accident injuries(67.20%)were the main cause of injury,with electric bicycle injuries accounting for the highest proportion(48.88%).In Schatzker classification,type I accounted for the highest proportion(31.19%),fllowed by type VI(19.06%).Conclusion The overall incidence of tibial plateau fractures is on the rise.The distributions of gender,age,side of lesion,admission season,injur
