对比经椎旁肌间隙入路与传统入路结合伤椎置钉治疗胸腰椎段脊柱骨折的疗效

摘要 目的 对比经椎旁肌间隙入路与传统入路结合伤椎置钉治疗胸腰椎段脊柱骨折的疗效。方法 应用医学研究观察法,选取2021年1月-2022年1月本院接收的50例胸腰椎段脊柱骨折患者的临床资料,按照治疗方法的不同,分为对照组(n=25例)和试验组(n=25例)。分别给予试验组椎旁肌间隙入路+伤椎置钉治疗,给予对照组传统入路+伤椎置钉治疗,就两组的治疗情况进行对比分析。结果 试验组总有效率优于对照组,两组数据差异具有统计学意义(P<0.05)。试验组患者手术时间、住院时间短于对照组,术中出血量少于对照组,组间差异有统计学意义(P<0.05)。试验组在生活事件量表LES评分指标(如<20分、<32分、>32分)上比之对照组的结果较明显,差异有统计学意义(P<0.05)。试验组在腰椎功能JOA指标(如主观症状和体征、日常活动受限度、膀胱功能、总评分)上比之对照组的结果差异较大,有统计学意义(P<0.05)。结论 胸腰椎段脊柱骨折治疗中经椎旁肌间隙入路结合伤椎置钉治疗的效果突出,对比传统入路结合伤椎置钉,前者有着更高的疗效,并在改善患者术后指标、生活事件量表LES评分和腰椎功能上效果更佳,值得在后续患者的治疗实践中大力推广实施。 Objective To compare effect of paravertebral muscle space approach and traditional approach combined with vertebral instrumentation for thoracolumbar spinal fractures.Methods The paper chose clinical data of 50 patients with thoracolumbar spine fractures admitted to our hospital from January 2021 to January 2022 with medical research observation method,and divided them into two groups based on different treatment methods:control group(n=25)and experimental group(n=25).Experimental group was treated with paravertebral muscle space approach and injured vertebral nail placement,while control group with traditional approach and injured vertebral nail placement.Curative effect between two groups was compared and analyzed.Results Clinical effect of experimental group was 92.00%(23/25),that of control group was 68.00%(17/25),with statistically significant difference between two groups(P<0.05).Experimental group and control group showed significant difference in average surgical time,intraoperative bleeding and hospital stay,with statistical significance(P<0.05).Experimental group showed significant difference in LES scores of<20 points,<32 points,and>32 points compared with control group,with statistical significance(P<0.05 for all).Experimental group showed significant difference in lumbar functional JOA indicators including subjective symptoms and signs,daily activity limits,bladder function,and total scores compared with control group,with statistical significance(P<0.05 for all).Conclusion Combination of paraspinal muscle space approach and injured vertebral nail placement can achieve significant effect for thoracolumbar spine fractures,the former has higher curative effect in improving postoperative indicators,prognosis life quality,and lumbar function than traditional approach and injured vertebral nail placement,which is worthy of great promotion and application in subsequent treatment practice of patients.
