关节镜下距腓前韧带+跟腓韧带重建术治疗踝关节外侧不稳的效果及对关节稳定性的影响

摘要 目的探讨关节镜下距腓前韧带+跟腓韧带重建术治疗踝关节外侧不稳的效果及对关节稳定性的影响。方法选取2019年6月至2022年6月我院收治的70例踝关节外侧不稳患者作为研究对象,按手术方式的差异将其分为对照组和观察组,各35例。对照组接受改良Brostrom手术治疗,观察组接受关节镜下距腓前韧带+跟腓韧带重建术治疗。比较两组的治疗效果。结果观察组的手术时间、术后发热时间长于对照组,伤口愈合时间、术后恢复正常活动时间短于对照组(P<0.05)。观察组的治疗优良率高于对照组(P<0.05)。术后6个月,观察组的距骨倾斜角小于对照组,距骨前移距离短于对照组(P<0.05)。术后6个月,观察组的疼痛、症状评分低于对照组,日常生活能力、运动及生活质量评分高于对照组(P<0.05)。结论关节镜下距腓前韧带+跟腓韧带重建术治疗踝关节外侧不稳效果显著,虽然所用手术时间更长,但患者术后恢复更快,且能改善关节稳定性,促进患者生活质量提升,值得推广。 Objective To investigate the effect of arthroscopic anterior talofibular ligament+calcaneofibular ligament reconstruction on lateral ankle instability and its influence on joint stability.Methods A total of 70 patients with lateral ankle instability admitted in our hospital from June 2019 to June 2022 were selected as the research objects.According to the difference of surgical methods,the patients were divided into control group and observation group,with 35 cases in each group.The control group received modified Brostrom surgery,and the observation group received arthroscopic anterior talofibular ligament+calcaneofibular ligament reconstruction.The therapeutic effects of the two groups were compared.Results The operation time and postoperative fever time of the observation group were longer than those of the control group,and the wound healing time and postoperative recovery time of normal activity were shorter than those of the control group(P<0.05).The excellent and good rate of treatment in the observation group was higher than that in the control group(P<0.05).At 6 months after operation,the talus inclination angle of the observation group was smaller than that of the control group,and the talus forward distance was shorter than that of the control group(P<0.05).At 6 months after operation,the scores of pain and symptom in the observation group were lower than those in the control group,and the scores of daily living ability,exercise and quality of life were higher than those in the control group(P<0.05).Conclusion Arthroscopic anterior talofibular ligament+calcaneofibular ligament reconstruction for the treatment of lateral ankle instability has significant effect.Although the operation time is longer,the patient recovers faster after surgery.The surgery can improve joint stability,and promote the quality of life of patients,which is worth promoting.
