摘要
目的 探讨分别用针状电刀、低温等离子辅助下鼻内镜下鼻前庭囊肿揭盖法和传统唇龈沟入路全切术的临床疗效及优缺点,术后冰敷对局部肿胀、疼痛的作用。方法 收集2021年6月—2022年9月福建省南平市第二医院行鼻前庭囊肿手术患者56例。根据不同手术方式进行分组,通过比较三组的出血量、手术时间、住院天数及住院费用。术后每组随机一半患者进行局部冰敷处理,来判断冰敷对术后疼痛和肿胀的作用。结果 三组的随访均无复发,手术效果明确,针状电刀组和低温等离子组出血量较传统入路组少,针状电刀组的费用较低温等离子组更低,差异有统计学意义(P <0.05)。术后研究组的面部肿胀及疼痛明显较未研究组减轻,差异有统计学意义(P <0.05)。结论 针状电刀辅助下鼻内镜下鼻前庭囊肿揭盖术治愈率高,出血量少且费用经济。护理方面局部冰敷对减轻术后肿胀及缓解疼痛有明显作用。
Objective To study the clinical efficacy,advantages and disadvantages of uncapping nasal vestibular cyst with needle electroknife and low temperature plasma assisted lower nasal endoscope and traditional total resection of labio-gingival groove approach,as well as the effect of postoperative ice on local swelling and pain.Methods A total of 56 patients who underwent nasal vestibular cyst surgery in No.2 Hospital of Nanping City,Fujian Province from June 2021 to September 2022 were collected.The patients were grouped according to different surgical methods.The amount of blood loss,operation time,hospital stay and hospitalization cost of the three groups were compared.After surgery,half of the patients in each group were randomLy treated with local ice treatment,so as to judge the effect of ice treatment on postoperative pain and swelling.Results Through the analysis of the three groups of data,no recurrence was observed in the follow-up of the three groups,and the surgical effect was clear.The amount of blood loss in the acicular electrotome group and the low-temperature plasma group was less than that in the traditional approach group,the cost of the acicular electrotome group was lower than that in the low-temperature plasma group,the difference were statistically significant(P<0.05).The facial swelling and pain in the resesrch group were significantly reduced than that in the control group,and the statistical analysis was significant(P<0.05).Conclusion The operation of uncovering nasal vestibular cyst under nasal endoscope assisted by acicular electroknife has high cure rate,less bleeding and economic cost,which is worthy of application.Local ice compress has obvious effect on reducing swelling and relieving pain after operation.
