非结核分枝杆菌肺病患者血清HMGB1和HMGN2的表达及诊断价值

Expressions of serum HMGB1 and HMGN2 in patients with nontuberculous mycobacteria-pulmonary disease and their diagnostic values

摘要 目的探讨非结核分枝杆菌肺病(NTM-PD)患者血清高迁移率族蛋白B1(HMGB1)和高迁移率族蛋白N2(HMGN2)的表达及诊断价值。方法选取NTM-PD患者96例为NTM组,肺结核病患者106例为结核组,100例健康者为对照组;收集研究对象一般资料,采用ELISA法检测NTM组和结核组患者入院后24 h、对照组体检当日血清中HMGB1和HMGN2水平;分别比较NTM组、结核组、对照组和不同菌种分型结核组患者的一般质料及血清HMGB1和HMGN2表达水平;采用受试者工作特征(ROC)曲线下面积(AUC)评估血清中HMGB1和HMGN2水平对NTM-PD的诊断价值;采用logistic回归分析影响NTM-PD发生的危险因素。结果与对照组比较,NTM组和结核组患者血清HMGB1、HMGN2表达均升高,且NTM组表达水平高于结核组,差异均有统计学意义(P<0.05);不同菌种分型结核组患者血清HMGB1和HMGN2表达水平比较,差异无统计学意义(P>0.05);NTM组患者肺内空洞、咳嗽、咯血、慢性阻塞性肺疾病、支气管扩张比均高于结核组,差异有统计学意义(P<0.05);ROC曲线显示,血清HMGB1和HMGN2水平诊断NTM-PD的AUC分别为0.902、0.872,对应的敏感度分别为80.21%、90.62%,特异度分别为89.62%、83.02%,二者联合诊断的AUC为0.896,敏感度为80.21%、特异度为91.51%;logistic回归分析结果显示,HMGB1和HMGN2、慢性阻塞性肺疾病、支气管扩张均是影响NTM-PD发生的危险因素(P<0.05)。结论NTM-PD患者血清中HMGB1和HMGN2表达均升高,二者联合检测可有效诊断NTM-PD疾病。 Objective To investigate the expressions of serum high mobility group protein B1(HMGB1)and high mobility group N2(HMGN2)in patients with nontuberculous mycobacteria-pulmonary disease(NTM-PD)and their clinically diagnostic values.Methods A total of 96 patients with NTM-PD were selected as NTM group and 106 patients with pulmonary tuberculosis as Tuberculosis group.One hundred healthy individuals were selected as control group.General data of three groups were collected.ELISA was used to detect the levels of serum HMGB1 and HMGN2 within 24 hours after admission in NTM and Tuberculosis groups and on the day of physical examination in control group.General data and the expression levels of serum HMGB1 and HMGN2 were compared among NTM group,Tuberculosis group,control group and the patients with different tuberculosis strain typing.Area under receiver operating characteristic(ROC)curve(AUC)was applied to assess the diagnostic values of serum HMGB1 and HMGN2 levels in NTM-PD.logistic regression was performed to analyze the risk factors affecting NTM-PD occurrence.Results When compared to control group,the expressions of serum HMGB1 and HMGN2 were increased in both NTM and Tuberculosis groups,and higher in NTM group than in Tuberculosis group(P<0.05).There were no statistically significant differences in the expression levels of serum HMGB1 and HMGN2 among tuberculosis patients with different tuberculosis strain typing(P>0.05).NTM group had significantly higher proportions of pulmonary cavity,cough,hemoptysis,chronic obstructive pulmonary disease and bronchodilation than those in Tuberculosis group(P<0.05).ROC analysis revealed that the AUC of serum HMGB1 and HMGN2 levels in diagnosing NTM-PD were 0.902 and 0.872,respectively,with corresponding sensitivities of 80.21%and 90.62%,specificity of 89.62%and 83.02%,respectively.The AUC of combined diagnosis of HMGB1 and HMGN2 was 0.896,with sensitivity of 80.21%and specificity of 91.51%.logistic regression analysis showed that HMGB1 and HMGN2,chronic obstructive pulmonary dis
