摘要
目的探讨星状神经节阻滞(SGB)对脊髓损伤(SCI)后中枢性疼痛(CP)大鼠疼痛行为的影响及机制。方法27只大鼠均分为假手术(Sham)组(仅切断T9-T11椎板并显露硬脊膜,但不损伤硬脊膜)、SCI组(行SCI)及SGB组(行SCI后予SGB),分别于术后第1天、第3天、第7天观察各组大鼠的行为学特征;分批处死各组大鼠取损伤段脊髓组织,采用后肢行为测试评分量表(BBB)评分和Hargreavest法检测各组大鼠后肢运动功能及热痛敏潜伏期(PWL),采用蛋白印迹技术和透射电镜检测各组大鼠术后第1天、第3天、第7天脊髓组织中微管相关蛋白轻链3(LC3)、家蚕隔离体蛋白1(p62)蛋白表达及自噬小体的表达。结果与Sham组相比,SCI组大鼠BBB评分、PWL持续下降(P<0.001),LC3、p62蛋白表达升高(P<0.005),透射电镜下脊髓可见少量自噬小体;与SCI组相比,SGB组大鼠自我伤害相对减轻,BBB评分、PWL明显升高(P<0.001),LC3蛋白表达升高、p62蛋白表达降低(P<0.005),透射电镜下脊髓可见大量自噬小体。结论SGB可缓解SCI后CP大鼠的疼痛行为,其机制可能与激活脊髓自噬有关。
Objective To investigate the effect and mechanism of stellate ganglion block(SGB)on pain behavior of rats with central pain(CP)after spinal cord injury(SCI).Methods Twenty-seven rats were divided into the sham operation group(only T9-T11 lamina was bitten off and dura mater was exposed,but dura mater was not damaged),the model group(spinal cord injury was performed)and the treatment group(stellate ganglion was severed after spinal cord injury).On the 1 st,3 rd,and 7 th days after operation,BBB score method was used to determine the recovery of movement,Hargreavest method was used to determine PWL,and the injured spinal cord tissues were applied to detect the autophagy-related proteins light chain 3(LC3)and sequestosome-1(p62)in spinal cord by Western blotting,and the expression of autophagosome in spinal cord was detected by transmission electron microscopy.Results Compared with the sham operation group,the rats in the model group showed different degrees of self-injury,while the latency of motor recovery and thermal pain leg contraction decreased continuously,the expression of LC3 and p62 increased(P<0.005),and a small amount of autophagosomes were observed in the spinal cord under transmission electron microscope.Compared with the model group,the self-injury of the rats in the treatment group was relatively reduced,and the latency of motor recovery and thermal pain leg contraction significantly increased(P<0.001).The expression of LC3 increased,the content of p62 protein decreased(P<0.005),and a large number of autophagosomes were observed in the spinal cord under transmission electron microscope.Conclusion SGB can alleviate the pain behavior of CP rats after SCI,and its mechanism may be related to the activation of autophagy in rat spinal cord.
