2017年-2021年内蒙古某院新生儿科住院患者疾病谱分析

Disease Spectrum Analysis of Inpatients in Neonatology Department of A Hospital in Inner Mongolia from 2017 to 2021

摘要 目的通过分析某三甲医院新生儿住院患者疾病谱的构成及变异状况,为新生儿的疾病防治提供参考依据。方法应用Excel 2016和SPSS26.0对2017年1月1日-2021年12月31日在内蒙古某院住院的新生儿病案首页信息进行回顾性统计分析,通过秩和比(RSR)及和谐系数(WR)对住院新生儿疾病谱顺位进行一致性检验分析,组间比较应用χ^(2)检验,P<0.05差异有统计学意义。结果5年间疾病谱顺位变化不显著,呈现高度一致性(WR=0.8381,χ^(2)=37.715,P=0.000)。其中,各季节疾病谱顺位变化不显著,存在一致性(WR=0.9273,χ^(2)=33.383,P=0.000)。年龄分布具有统计学差异的为早产儿(χ^(2)=81.474,P=0.000)、新生儿感染(χ^(2)=27.441,P=0.000)、新生儿肺炎(χ^(2)=134.189,P=0.000),新生儿脐炎(χ^(2)=4.303,P=0.033)、新生儿败血症(χ^(2)=30.005,P=0.000)。单胎或多胎对住院新生儿疾病谱分布具有统计学差异的为早产儿(χ^(2)=6.374,P=0.006)、新生儿呼吸窘迫综合征(χ^(2)=101.838,P=0.000)、新生儿低血糖症(χ^(2)=11.117,P=0.000)、新生儿缺氧缺血性脑病(χ^(2)=9.271,P=0.000)。结论新生儿高胆红素血症、新生儿肺炎、新生儿感染、早产儿、新生儿呼吸窘迫综合征等是新生儿科高发疾病,应有针对性地加强上述疾病的诊治管理,优化医疗资源配置。 Objective In order to reveal the disease spectrum and provide reference for clinical application and prevention,the composition and variation of disease spectrum for neonatal inpatients in a 3A grade hospitalwas analyzed in this study.Methods A retrospectively analyze of the data of neonatal cases hospitalized in a hospital in Inner Mongolia from 2017 to 2021 was conducted by Excel 2019 and SPSS26.0 software.The consistency test on disease spectrum sequence of hospitalized newborns from 2017 to 2021was completed by using the rank sum ratio(RSR)and concordant coefficient(WR)for the significance at the 0.05 level.Besides,χ^(2) testwas used for the comparison among different groups.Results The results showed that the variation of disease spectrum during 5 years was not significant(WR=0.8381,χ^(2)=37.715,P=0.000).There was no statistical difference in seasonal distribution(WR=0.9273,χ^(2)=33.383,P=0.000).Statistically significant was generated to the preterm infants(χ^(2)=81.474,P=0.000),neonatal infection(χ^(2)=27.441,P=0.000),neonatal pneumonia(χ^(2)=134.189,P=0.000),neonatal omphalitis(χ^(2)=4.303,P=0.033),and neonatal omphalitis(χ^(2)=4.303,P=0.033)determined by age at the 0.05 level.Additionally,a statistically significant was observed to the premature infants(χ^(2)=6.374,P=0.006),neonatal respiratory distress syndrome of newborn(χ^(2)=101.838,P=0.000),neonatal hypoglycemia of newborn(χ^(2)=11.117,P=0.000)and neonatal hypoxic ischemic encephalopathy(χ^(2)=9.271,P=0.000)determined by single or multiple birthsat the 0.05 level.Conclusions It was the neonatal the high incidence disease of neonatal hospitalization,hyperbilirubinemia,neonatal pneumonia,neonatal infection,premature infants and neonatal respiratory distress syndrome.We should strengthen the diagnosis and treatment management of the above diseases,optimize the allocation of medical resources.
