摘要
目的探讨颈椎病患者住院费用的影响因素,为控制颈椎病医疗费用,减轻患者经济负担提供参考。方法从某综合医院筛选2018年1月1日-2022年12月31日主要诊断为颈椎病的住院病案首页信息,采用非参数检验、相关性分析、多重线性回归和通径分析模型探索住院费用的影响因素。结果共收集992例颈椎病患者费用数据,男性293例,女性629例,平均年龄54.13±13.70岁。颈椎病治疗患者的住院天数四分位数为9(7-13)天。住院费用四分位数为5070.66(3785.18-6826.61)元,排名前3的费用占比为治疗费(60.28%)、检查检验费(21.76%)、医疗服务费(7.11%)。住院总费用受到住院天数、年龄、颈椎病分型等多个因素影响,其中住院天数是直接影响因素和主要影响因素,其效应量为0.838。性别和支付方式是间接影响因素,年龄、颈椎病分型和药占比等不仅直接影响住院费用,还通过住院天数间接影响住院费用。结论颈椎病患者住院费用主要受住院天数影响,治疗费占比最高。缩短住院天数,合理控制治疗费,可有效降低颈椎病治疗患者的住院费用。
Objectives This study aims to observe the influencing factors of the hospitalization expenses of patients with cervical spondylosis and provide a reference for controlling the medical expenses of this disease.Methods The front-page information of inpatient medical records mainly diagnosed with cervical spondylosis from January 1,2018 to December 31,2022 was screened from a general hospital.Non-parametric tests,correlation analysis,multiple linear regression,and path analysis models were used to explore the influencing factors of inpatient expenses.Results A total of 992 patients with cervical spondylosis were collected,including 293 males and 629 females,with an average age of 54.13±13.70 years old.The quartile of hospitalization days for patients treated for cervical spondylosis was 9(7-13)days.The inpatient expenses quartile was 5070.66(3,785.18-6826.61)yuan,and the top 3 expenses were treatment fees(60.28%),examination fees(21.76%),and medical service fees(7.11%).The total cost of hospitalization was affected by several factors,such as length of stay,age,and classification of cervical spondylosis,among which length of stay was the direct and main influencing factor,with an effect size of 0.838.Gender and payment methods were indirect influencing factors.Age,type of cervical spondylosis,and drug proportion not only directly affected hospitalization costs,but also indirectly affected hospitalization costs through hospitalization days.Conclusions The hospitalization expenses of patients with cervical spondylosis are mainly affected by the length of hospitalization,and the treatment expenses account for the highest proportion.Shortening the length of hospitalization and reasonably controlling treatment costs can effectively reduce hospitalization costs for patients with cervical spondylosis.
