摘要
肿瘤相关性失眠是指发生在肿瘤患者中的睡眠障碍,常继发于肿瘤疾病且与肿瘤疾病本身相关或与肿瘤治疗相关的失眠症状,严重影响肿瘤患者的生存质量。林胜友主任中医师基于“阴火论”的理论基础,结合肿瘤疾病发生发展及疾病诊治过程中的疾病特征,提出肿瘤相关性失眠病因病机复杂,应根据患者肿瘤疾病变化不同阶段的病症表现特点,辨病、辨证结合论治。总览疾病全局,其病机总属脾胃(正气)亏虚,阴阳失衡,病程虚、火夹杂变换。基于“阴火论”从脾论治,以“补益中气,甘温除热”为治则,佐以宁心安神。以补中益气为组方基础,随证加减,兼顾治疗肿瘤疾病过程中出现的各类兼证、变证,灵活变通,以达阴平阳秘。本文从该病的病因病机、结合疾病特点的论治方法、特色用药和病案举隅进行阐释,以期为中医辨治肿瘤相关性失眠提供新的思路和方法。
Tumor-related insomnia refers to the sleep disorder occurring in tumor patients,which is often secondary to the tumor disease and related to the tumor disease itself or related to tumor treatment,and seriously affects the quality of life of tumor patients.Director Lin Shengyou,a traditional Chinese medicine practitioner,based on the theoretical foundation of the"Yin Fire Theory"and combined with the disease characteristics during the occurrence,development,diagnosis,and treatment of tumor diseases,proposed that the etiology and pathogenesis of tumor-related insomnia are complex,and shall be treated through a combination of disease differentiation and syndrome differentiation based on the different stages of tumor disease changes in patients.Taking a comprehensive view of the overall situation of the disease,its pathogenesis always belongs to deficiency of the spleen and stomach(vital energy),imbalance of yin and yang,and mixed transformation of deficiency and fire during the disease course.Based on the"Yin Fire Theory",the treatment is based on the spleen,with the principle of"tonifying and supplementing the deficiency,clearing heat with sweet-warm drugs"as the treatment principle,supplemented by calming the heart and calming the mind.Based on tonifying and supplementing the deficiency as the foundation of the formula,adding and subtracting according to the symptoms,taking into account various concurrent and variable symptoms that may occur during the treatment of tumor diseases,and flexibly adapting to achieve relative equilibrium of Yin-yang.This article explains the etiology and pathogenesis of the disease,combined with the characteristics of the disease’s treatment methods,characteristic drugs,and medical records,in order to provide new ideas and methods for the differentiation and treatment of tumor-related insomnia in traditional Chinese medicine.
