摘要
目的探究胆总管结石患者采用内镜下小气囊乳头扩张联合网篮碎石取石术(EPBD)治疗的临床研究。方法随机抽取2019年1月~2021年7月本院收治的胆总管结石患者72例纳入研究对象,将所有研究对象根据数字表法分为对照组(n=36)、实验组(n=36),对照组患者开展内镜下乳头括约肌切开术(EST)治疗,实验组开展EPBD治疗,比较两组取石成功率、近期及远期并发症情况。结果两组患者取石成功率近似,无统计学意义(P>0.05);近期并发症中,高淀粉酶血症、乳头出血、十二指肠穿孔、PEP发生率组间均无统计学差异(P>0.05),实验组胆道感染率较对照组明显降低,差异有统计学意义(P<0.05);两组间反流性胆管炎发生率无统计学差异(P>0.05),结石复发率实验组明显低于对照组,有统计学意义(P<0.05)。结论胆总管结石采用EPBD治疗可有效降低胆道感染,针结石复发具有较大的干预效果。
Objective To investigate the clinical study of endoscopic small balloon papillary expansion combined with basket lithotripsy(EPBD)in the treatment of patients with common bile duct stones.Methods A total of 72 patients with common bile duct stones who were treated in our hospital from January 2019 to July 2021 were randomly selected and in⁃cluded in the research subjects,All the subjects were randomly divided into control group and experimental group,control group(n=36),experimental group(n=36),and control group patients.Endoscopic sphincterotomy(EST)treatment was carried out,and EPBD treatment was carried out in the experimental group.The success rate of stone removal and the short⁃term and long⁃term complications were compared between the two groups.Results The success rate of stone removal be⁃tween the two groups was similar,and there was no statistical significance(P>0.05).Among the recent complications,there was no significant difference in the incidence of hyperamylase,nipple bleeding,duodenal perforation,and PEP between the groups(P>0.05).the biliary infection rate in the experimental group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05);there was no significant difference in the incidence of reflux cholangitis between the two groups(P>0.05),and the recurrence rate of stones The experimental group was significantly lower than the control group,with statistical significance(P<0.05).Conclusion EPBD treatment of common bile duct stones can effectively reduce biliary tract infection,and acupuncture has a greater intervention effect on recurrence of stones.
