摘要
目的 研究超声弹性成像面积比值法(Elastography imaging area ratio, EIAR)联合血清甲状腺过氧化物酶抗体(Thyroid peroxidase antibody, TPO-Ab)在甲状腺结节性质辨别中的应用价值。方法 选取2019年6月-2021年10月期间我院收治的甲状腺结节患者82例,根据病理诊断结果分为良性结节组51例,和恶性结节组31例,在治疗前分别使用超声弹性成像面积比值法和血清TPO-Ab检测法对甲状腺结节的性质进行预测,将预测结果与病理检测结果对比,分析两者联合检测的诊断价值。结果 良性组患者的EIAR面积比值为1.22±0.39,显著低于恶性组1.73±0.46,两组对比差异具有统计学意义(P<0.05)。对EIAR、血清TPO-Ab检测和两者联合检测三者对恶性结节的诊断效能做检测,EIAR的敏感度为80.4%特异度为87.1%,AUC为0.837;血清TPO-Ab检测的敏感度为74.5%,特异度为80.6%,AUC为0.776;联合检测的敏感度为96.1%,特异度为96.8%,AUC为0.964;联合检测的敏感度>EIAR>血清TPO-Ab检测(Z=17.552,P<0.001),联合检测的特异度>EIAR>血清TPO-Ab检测(Z=13.781,P<0.001);联合检测的AUC>EIAR>血清TPO-Ab检测(Z=6.728,P<0.001)。结论 EIAR联合血清TPO-Ab对甲状腺结节的性质预测具备良好特异度、敏感度,能够有效帮助进行甲状腺的性质诊断。
Objective To study the application value of ultrasound elastography imaging area ratio(eiar)combined with serum thyroid peroxidase antibody(TPO AB)in identifying the nature of thyroid nodules.Methods 82 patients with thyroid nodules treated in our hospital from June 2019 to October 2021 were selected.According to the pathological diagnosis results,they were divided into 51 cases in benign nodule group and 31 cases in malignant nodule group.Before treatment,the properties of thyroid nodules were predicted by Ultrasonic Elastic imaging area ratio method and serum TPO AB detection method respectively.The predicted results were compared with the pathological detection results,and the diagnostic value of the combined detection of the two was analyzed.Results The area ratio of eiar in benign group was 1.22±0.39,which was significantly lower than that in malignant group(1.73±0.46).The difference between the two groups was statistically significant(P<0.05).The sensitivity of eiar was 80.4%,the specificity was 87.1%,and the AUC was 0.837;The sensitivity,specificity and AUC of serum TPO AB were 74.5%,80.6%and 0.776 respectively;The sensitivity,specificity and AUC of combined detection were 96.1%,96.8%and 0.964 respectively;The sensitivity of combined detection was>eiar>serum TPO AB detection(z=17.552,P<0.001),and the specificity of combined detection was>eiar>serum TPO AB detection(z=13.781,P<0.001);AUC>eiar>serum TPO AB(z=6.728,P<0.001).Conclusion Eiar combined with serum TPO AB has good specificity and sensitivity in predicting the nature of thyroid nodules,and can effectively help to diagnose the nature of thyroid.
