摘要
目的设计适用于乳腺癌改良根治术后患者的自我管理记录单并探讨其应用效果。方法以自我效能理论为理论基础,采用专家会议法设计出由健康问题描述、自我管理方式和效果评价3个项目组成的自我管理记录单,应用于2021年3月-2022年3月江西省肿瘤医院的86例接受乳腺癌改良根治术的女性患者,按入院时间顺序将所有患者根据随机数字表分为对照组和观察组,各43例,观察组在常规术后护理方案的基础上,指导患者使用自我管理记录单,对照组采用常规的术后护理方案。比较2组患者在术后第3天、第4次化疗后、第8次化疗后的自我管理效能感水平、上肢日常生活功能、生命质量状况。结果观察组患者的自我管理效能感水平、上肢日常生活功能、生命质量状况在干预后得分高于对照组,差异有统计学意义(P<0.05),且随着干预时间的延长,效果逐渐明显(P<0.05)。结论使用自我管理记录单有助于提高乳腺癌改良根治术后患者的自我管理效能、上肢功能水平,改善其生命质量状况。
Objective To design a self-management record sheet for patients after modified radical resection of breast cancer and to explore its application effect.Methods Based on the theory of self-efficacy,a self-management record composed of 3 items,health problem description,self-management method,and effect evaluation,which was designed by the expert meeting method,was applied to 86 female patients who underwent modified radical mastectomy in Jiangxi Provincial Cancer Hospital from March 2021 to March 2022.According to the time order of admission,all patients were divided into the control group and observation group in 43 cases by random number table,and the observation group was based on the routine postoperative care plan which guided the patient to use the self-management record list,and the control group was treated with a conventional postoperative care regimen.The self-management efficacy,upper limb daily life function,and quality of life status between the two groups were compared after the 3 rd postoperative day,the 4 th chemotherapy,and the 8th chemotherapy.Results The scores of self-management efficacy,upper limb daily life function,and quality of life status in the observation group were higher than those in the control group after intervention,and the differences were statistically significant(P<0.05),and the effects were gradually obvious with the extension of intervention time(P<0.05).Conclusions The use of self-management record sheet can improve the self-management efficiency,upper limb function level,and quality of life of patients after modified radical breast cancer surgery.
