摘要
盆腔炎指女性上生殖道及周围组织的炎症,可发生在一个部位,也可发生于多个部位,常见有输卵管炎、输卵管卵巢炎。若得不到及时治疗,容易引起败血症、弥漫性腹膜炎、感染性休克,直接威胁患者的生命安全。慢性盆腔炎发生主要原因为免疫因素、病情迁移、衣原体感染、病理改变、产后、流产后以及妇科手术后、下生殖道感染、性卫生不良等,主要临床表现为慢性盆腔痛、不孕及异位妊娠、月经异常、子宫内膜炎、子宫多后倾、活动受限或粘连固定。临床主要检查方法为超声检查、子宫输卵管碘油造影、组织病理学检查、血常规检查、阴道分泌物检查、肿瘤标记物检查、聚合酶链反应检测。中医认为盆腔炎属于“带下病、产后发热”范畴。近年来,中医分别从内治、外治两方面治疗慢性盆腔炎,且取得了一定的效果。本文现将慢性盆腔炎的中医治疗进展进行综述,以期为慢性盆腔炎的临床治疗提供参考方案。
Pelvic inflammatory disease refers to the inflammation of the upper reproductive tract and surrounding tissues in women,which can occur in one area or multiple areas,commonly including salpingitis and salpingo-ovarianitis.If not treated in a timely manner,it can easily lead to sepsis,diffuse peritonitis,and septic shock,directly threatening the patient's life safety.The main causes of chronic pelvic inflammatory disease are immune factors,disease migration,chlamydia infection,pathological changes,postpartum,post abortion and after gynecological surgery,lower reproductive tract infection,poor sexual hygiene,etc.The main clinical manifestations are chronic pelvic pain,infertility and ectopic pregnancy,menstrual abnormalities,endometritis,and multiple posterior leaning of the uterus,limited mobility or adhesion fixation.The main clinical examination methods include ultrasound examination,hystero-salpingography,histopathological examination,blood routine examination,vaginal secretion examination,tumor marker examination and polymerase chain reaction detection.Traditional Chinese medicine believes that pelvic inflammatory disease belongs to the category of"vaginal discharge disease and postpartum fever".In recent years,traditional Chinese medicine has treated chronic pelvic inflammatory disease from both internal and external perspectives,and has achieved certain effect.This article reviews the progress of traditional Chinese medicine treatment for chronic pelvic inflammatory disease,in order to provide a reference plan for the clinical treatment of chronic pelvic inflammatory disease.
