软式内镜床旁预处理自动注液装置应用效果研究

摘要 目的 探究软式内镜床旁预处理自动注液装置与传统床旁预处理方法在软式内镜洗消过程应用效果对比。方法 选取2019年5月—2021年6月于江苏省人民医院(南京医科大学第一附属医院)急诊科就诊的875例患者所用的软式内镜入组作为研究对象,依据患者软式内镜检查的时间分为对照组和试验组。对照组为2019年5月—2020年5月使用传统酶液桶进行预处理内镜诊治患者使用的内镜,共有421根;试验组为2020年6月—2021年6月使用“软式内镜床旁预处理自动注液装置”进行预处理进行内镜诊治患者使用的内镜,共有454根。在患者取出内镜后,进行三磷酸腺苷(ATP)生物荧光检测和残留蛋白检测,随后按照《软式内镜清洗消毒技术规范》中预处理流程在诊疗室直接进行床旁预处理。床旁预处理结束后,再次进行三磷酸腺苷生物荧光检测和残留蛋白检测,并统计对照组与试验组所有软式内镜预处理中的酶液使用量、预处理操作时间。结果 对照组在床旁预处理后的ATP水平为(828.10±62.36)RLU,清洗消毒后的ATP水平为(94.12±26.90)RLU,显著高于试验组预处理后的(513.78±59.41)RLU和清洗消毒后的(44.67±16.52)RLU,对照组预处理后微生物残留率也显著高于试验组(P>0.05)。而对照组和试验组在预处理前后以及清洗消毒后的残余蛋白检测均未发现显著差异(P>0.05)。对照组在床旁预处理时酶液使用量为(121.43±17.23)mL,显著高于试验组的(100±0.00)mL;且对照组的预处理时间(3.69±0.55)min也明显长于试验组(2.25±0.53)min(P>0.05)。对照组和试验组两组在清洗消毒后的清洗消毒合格率无明显差异(P>0.05)。结论软式内镜床旁预处理自动注液装置为床旁预处理操作提供了便利,能实现全自动出杯落杯,一用一换定量使用清洗液,避免清洗液浪费,提高床旁预处理效率,防止交叉感染,在临床环境中使用安全可靠,值得推广。 Objective To investigate the effect of soft endoscopy bedside pre-treatment automatic fluid injection device compared with traditional bedside pre-treatment method in the process of soft endoscopy decontamination.Methods A group of 875 emergency patients attending the Department of Gastroenterology at Jiangsu Provincial People's Hospital(The First Affiliated hospital of Nanjing Medical University)from May 2019 to June 2021 was selected as the study population and divided into a control group and a test group based on the time of suffering from soft endoscopy.The control group consisted of 421 endoscopes used by patients who underwent endoscopic diagnosis and treatment with traditional enzyme solution barrel for pretreatment from May 2019 to May 2020.The experimental group consisted of 454 endoscopes used by patients who underwent endoscopic diagnosis and treatment with“automatic infusion device for bedside pretreatment of flexible endoscope”for pretreatment from June 2020 to June 2021.After removal of the endoscope from the patient,a biofluorescence test for Adenosine triphosphate(ATP)and a residual protein test are performed,followed by a direct bedside pre-treatment in the consultation room according to the pre-treatment procedure in the Technical Specification for Cleaning and Disinfection of Flexible Endoscopes.After the bedside pre-treatment,adenosine triphosphate biofluorescence test and residual protein test were performed again,and the amount of enzyme solution used in all flexible endoscopic pre-treatments,the pre-treatment time.Results The ATP level of the control group after bedside pretreatment was(828.10±62.36)RLU,and the ATP level after cleaning and disinfection was(94.12±26.90)RLU,which were significantly higher than those of the experimental group after pretreatment(513.78±59.41)RLU and after cleaning and disinfection(44.67±16.52)RLU,respectively.The microbial residue rate of the control group after pretreatment was also significantly higher than that of the experimental group,P<0.05.Howev
