摘要
目的探究弥散张量成像联合血栓弹力图(TEG)对缺血性脑卒中的诊断价值。方法回顾性分析2020年4月至2021年4月期间于医院就诊的134例疑似缺血性脑卒中患者的临床资料,以数字减影血管造影(DSA)检查结果为金标准,最终确诊缺血性脑卒中98例(试验组)、非缺血性脑卒中36例(对照组)。比较两组弥散张量成像和TEG参数。结果试验组平均弥散率(MD)值、表观弥散系数(ADC)值、最大振幅(MA)值、弹力图最大切角(Angle角)、凝血指数均高于对照组,各向异性分数(FA)值、反应时间(R)值、凝固时间(K)值均低于对照组(P<0.05)。弥散张量成像联合TEG诊断缺血性脑卒中的特异度、准确度均高于弥散张量成像、TEG单项诊断,差异有统计学意义(P<0.05)。结论弥散张量成像能够敏感地发现病变,客观地测量FA、ADC值,将病变量化,TEG参数能够及时地反映机体凝血状态变化。弥散张量成像联合TEG可有效提高缺血性脑卒中的诊断效能。
Objective To explore the diagnostic value of diffusion tensor imaging combined with thromboelastography(TEG)in ischemic stroke.Methods The clinical data of 134 patients with suspected ischemic stroke treated in the hospital from April 2020 to April 2021 were analyzed retrospectively,and the results of digital subtraction angiography(DSA)were taken as the gold standard,and 98 cases of ischemic stroke(test group)and 36 cases of non-ischemic stroke(control group)were finally confirmed.Two groups were compared for diffusion tensor imaging and TEG parameters.Results The mean diffusion rate(MD)value,apparent diffusion coefficient(ADC)value,maximum amplitude(MA)value,maximum cut angle(Angle angle),and coagulation index of the test group were all higher than those of the control group,and the anisotropic fraction(FA)value,reaction time(R)value,and coagulation time(K)value of the test group were all lower than those of the control group,and the difference was statistically significant(P<0.05).The specificity and accuracy of diffusion tensor imaging and TEG of the test group were higher than those of diffusion tensor imaging and TEG,and the difference was statistically significant(P<0.05).Conclusions Diffusion tensor imaging can sensitively detect lesions,objectively measure FA and ADC values,quantify lesions,and TEG parameters can reflect the changes of body coagulation status in time.Diffusion tensor imaging combined with TEG can effectively improve the diagnostic efficacy of ischemic stroke.
