摘要
目的分析肌骨超声检查用于鉴别诊断创伤性浅表软组织损伤的准确性。方法选取2021年6月至2023年5月南阳市中心医院收治的300例创伤性浅表软组织损伤患者,全部患者均接受磁共振成像检查和肌骨超声检查,分析患者肌骨超声检查的影像学特点,将手术病理检查结果当成金标准,比较患者磁共振成像检查、肌骨超声检查的准确性、特异度、灵敏度。结果全部300例患者中,肌骨超声检查共准确诊断260例,其中包括25例实性肿块类、143例囊性肿块类、83例软组织撕裂、9例神经损伤。将手术病理检查结果当成诊断金标准,在实性肿块、囊性肿块、肌腱撕裂、韧带撕裂、神经损伤的检查准确性方面,磁共振成像检查与肌骨超声检查比较,差异无统计学意义(P>0.05);而肌骨超声对半月板撕裂的检查准确性显著低于磁共振成像检查,差异有统计学意义(P<0.05);磁共振成像检查与肌骨超声检查的特异度及灵敏度比较,差异无统计学意义(P>0.05)。结论在对创伤性浅表软组织损伤进行鉴别诊断时,肌骨超声检查不仅具有较强的可重复性,而且无创安全、能动态观察,能取得与磁共振成像检查类似的准确性,然而在对半月板撕裂进行诊断时,肌骨超声检查的准确性较低。
Objective To analyze the accuracy of musculoskeletal ultrasonography in differential diagnosis of traumatic superficial soft tissue injury.Methods A total of 300 patients with traumatic superficial soft tissue injury admitted to our hospital from June 2021 to May 2023 were selected.All patients underwent magnetic resonance imaging and musculoskeletal ultrasonography.The imaging characteristics of musculoskeletal ultrasonography were analyzed,and theResults of surgical pathologic examination were taken as the gold standard.The accuracy,specificity and sensitivity of MRI and musculoskeletal ultrasound were compared.Results Among the 300 patients,260 cases were diagnosed accurately by musculoskeletal ultrasound,including 25 cases of solid mass,143 cases of cystic mass,83 cases of soft tissue tear and 9 cases of nerve injury.Taking theResults of surgical pathology as the gold standard for diagnosis,there was no significant difference between MRI and musculoskeletal ultrasonography in the accuracy of solid mass,cystic mass,tendon tear,ligament tear and nerve injury(P>0.05).Conclusion In the differential diagnosis of traumatic superficial soft tissue injury,musculoskeletal ultrasonography not only has strong repeatability,but also is non-invasive,safe,dynamic observation,and can achieve similar accuracy to MRI.However,in the diagnosis of meniscus tear,musculoskeletal ultrasonography has low accuracy.
