摘要
目的探讨儿童重症监护室(PICU)肺出血患儿死亡的危险因素及干预对策。方法回顾性分析2021年8月至2023年2月在商丘市第一人民医院接受治疗的PICU肺出血患儿共计73例,统计患儿死亡情况,并进行单因素分析与Logistic多因素回归分析。结果73例PICU肺出血患儿出现死亡48例,发生率为65.75%;死亡组PaO_(2)<100mmHg、Na^(+)<135 mmol/L、HGB≤70 g/L、PLT≤130×10^(9)/L、PCIS<70分、发生休克患儿占比与生存组相比更高,差异有统计学意义(P<0.05);Logistic多因素分析结果显示,PaO_(2)<100 mmHg(OR=1.856)、Na^(+)<135mmol/L(OR=1.993)、HGB≤70 g/L(OR=3.085)、PLT≤130×10^(9)/L(OR=2.132)、PCIS<70分(OR=2.004)、发生休克(OR=2.051)是影响PICU肺出血患儿出现死亡的独立危险因素,差异有统计学意义(P<0.05)。结论肺出血患儿在PICU中死亡率较高,其发生风险与低水平PaO_(2)、Na^(+)、HGB、PLT、PCIS及休克等有关,临床需要实施有效的干预对策,以降低死亡率。
Objective To investigate the risk factors and intervention strategies for the death of children with pulmonary hemorrhage in child intensive care unit(PICU).Methods A total of 73 children with PICU pulmonary hemorrhage who received treatment in our hospital from August 2021 to February 2023 were retrospectively analyzed.The death of the children was statistically analyzed,and univariate analysis and Logistic multivariate regression analysis were performed.Results Among the 73 children with pulmonary hemorrhage in PICU,48 cases died,and the incidence was 65.75%.PaO_(2)<100 mmHg,Na^(+)<135 mmol/L,HGB≤70 g/L,PLT≤130×10^(9)/L,PCIS<70,and the proportion of children with shock in the death group were higher than those in the survival group,with statistical significance(P<0.05).Logistic multivariate analysisResults showed that PaO_(2)<100 mmHg(OR=1.856),Na^(+)<135 mmol/L(OR=1.993),HGB≤70 g/L(OR=3.085),PLT≤130×10^(9)/L(OR=2.132),PCIS<70(OR=2.004)and shock(OR=2.051)were independent risk factors for death in children with PICU pulmonary hemorrhage,and the difference was statistically significant(P<0.05).Conclusion The mortality rate of children with pulmonary hemorrhage in PICU is higher,and its risk is related to low levels of PaO_(2),Na^(+),HGB,PLT,PCIS and shock,etc.Effective intervention strategies should be implemented to reduce the mortality.
