摘要
目的:探讨妊娠期糖尿病(GDM)合并妊娠期高血压疾病患者血清过氧化物氧化还原酶1(PRDX1)、晚期氧化蛋白产物(AOPP)水平与胰岛素抵抗(IR)关系。方法:选择2020年3月-2022年3月本院妇产科治疗的GDM合并妊娠期高血压疾病孕妇103例作为合并症组,单纯GDM孕妇110例作为GDM组,血压血糖均正常孕妇118例作为对照组。酶联免疫吸附法检测血清PRDX1、AOPP水平,计算胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)。采用Pearson相关分析GDM合并妊娠期高血压疾病患者血清PRDX1、AOPP与IR关系,受试者工作特性曲线(ROC)评估血清PRDX1、AOPP对GDM合并妊娠期高血压疾病的预测价值。结果:对照组、GDM组、合并症组血清PRDX1、AOPP、FPG、FINS、HOMA-IR水平依次升高,HOMA-β依次降低(均P<0.05)。Pearson相关分析,GDM合并妊娠期高血压疾病孕妇血清PRDX1、AOPP与HOMA-IR均呈正相关,与HOMA-β均呈负相关(均P<0.05)。ROC曲线分析,预测GDM合并妊娠期高血压疾病,血清PRDX1的曲线下面积(AUC)为0.778,截断值36.25ng/L,敏感度92.6%、特异度56.5%;血清AOPP AUC为0.862,截断值13.24g/L,敏感度92.6%、特异度66.2%,二者联合的AUC为0.901,敏感度87.7%、特异度86.5%。结论:GDM合并妊娠期高血压疾病患者血清PRDX1、AOPP水平异常升高,二者水平与IR相关,且可作为预测GDM孕妇合并妊娠期高血压疾病的有效指标。
Objective:To explore the correlation between the serum peroxiredoxin 1(PRDX1)and advanced oxidation protein products(AOPP)levels of pregnant women with gestational diabetes mellitus(GDM)complicated with gestational hypertension disorders(GHD)and their insulin resistance.Methods:A total of 103 pregnant women with GDM complicated with GHD were selected in group A,110 pregnant women with GDM alone were selected in group B,and 118 pregnant women with normal blood pressure and blood glucose were selected in group C from March 2020 to March 2022.The levels of serum PRDX1 and AOPP of the women in the three groups were detected by enzyme-linked immunosorbent assay.The homeostatic model assessment(HOMA)for insulin resistance(IR)and HOMA for isletβ(HOMA-β)of the women in the three groups were calculated.Pearson correlation analysis was used to explore the correlation between the serum PRDX1 and AOPP levels of the women with GDM complicated with GHD and their IR.Receiver operator characteristic(ROC)curve was used to evaluate the value of the serum PRDX1 and AOPP levels of the pregnant women for predicting their GDM complicated with GHD.Results:The levels of serum PRDX1,AOPP,FPG,and FINS,and the HOMA-IR value of the women in group C,in group B,and in group A had increased gradually,while the HOMA-β value of the women in group C,in group B,and in group A had decreased gradually(all P<0.05).Pearson correlation analysis showed that the serum PRDX1 and AOPP levels of the pregnant women with GDM complicated with GHD were positively correlated with their HOMA-IR value,and were negatively correlated with their HOMA-value(all P<0.05).ROC curve analysis showed that the area under the curve(AUC),the cutoff value,the sensitivity,and the specificity of serum PRDX1level of the women for predicting their GDM complicated with GHD were 0.778,36.25ng/L,92.6%,and 56.5%,respectively.The AUC,the cutoff value,the sensitivity,and the specificity of serum AOPP level of the women for predicting their GDM complicated with GHD were 0.862,13.24g/L,
