摘要
目的:观察多模式干预对老年认知功能减退患者脑网络重塑的影响。方法:2020年1月—2023年1月,选取上海市静安区中心医院收治的老年认知功能减退患者120例作为研究对象。采用随机数字表法将其分为观察组和对照组,每组各60例。对照组:对患者实施常规认知康复训练;观察组:对患者实施多模式干预。干预时间为3个月。比较两组患者干预前后认知功能评分、心理状况、脑脊液生物标志物、脑网络结构和功能等指标。结果:干预后,两组患者智能状态检查量表评分、蒙特利尔认知评估量表评分、任务相关功能性磁共振成像激活强度、静息态功能性磁共振成像聚类系数、分数各向异性值等指标均高于干预前,且观察组高于对照组;观察组患者焦虑自评量表、抑郁自评量表评分低于干预前,对照组患者评分较干预前无显著变化;两组患者脑脊液β-淀粉样蛋白、微管结合蛋白水平低于干预前,且观察组低于对照组。结论:实施多模式干预可以有效改善老年认知功能减退患者的心理状态,促进脑网络重塑,有利于患者认知功能的恢复。
OBJECTIVE To observe the effect of multimodal intervention on brain network remodeling in elderly patients with cognitive impairment.METHODS A total of 120 elderly patients with cognitive impairment from January 2020 to January 2023 in Shanghai Jing'an District Central Hospital were included,60 in the observation group and 60 in the control group.The control group received routine cognitive rehabilitation training,and the observation group received multimodal intervention.The cognitive function scores,psychological status,cerebrospinal fluid markers and brain network structure and function were compared between the two groups before and after intervention.RESULTS The Mini-mental State Examination,Montreal Cognitive Assessment) score,task-related Functional magnetic resonance imaging activation intensity,re sting-state functional magnetic resonance imaging clustering coefficient andfractional anisotropy of the two groups after intervention were higher than those before intervention,and the observation group was higher than the control group.After the intervention,the SAS(self-rating anxiety scale) and SDS(Self-Rating Depression Scale) scores of the observation group were lower than those before the intervention,and there was no significant change in the controltau(microtubule-binding protein) in the two groups after intervention were lower group.The levels of cerebrospinal fluid β-amyloid protein and than those before intervention,and the observationgroup was lower than the control group.CONCLUSION The implementation of multi-mode intervention can effectively improve the psychological state of elderly patients with cognitive impairment,promote the remodeling of brain network,and thus facilitate the recovery of cognitive function.
