摘要
目的:观察超声引导下射频消融术联合聚多卡醇局部注射治疗下肢静脉曲张患者的效果。方法:选取2020年1月至2023年1月该院收治的150例下肢静脉曲张患者进行前瞻性研究,按照随机数字表法将其分为观察组和对照组各75例。对照组实施超声引导下聚多卡醇局部注射治疗,观察组在对照组基础上联合射频消融术治疗,两组治疗后均随访3个月。比较两组治疗前后静脉临床症状严重程度评分表(VCSS)评分,治疗前后血清学指标[白细胞介素-2(IL-2)、白细胞介素-10(IL-10)、基质金属蛋白酶组织抑制因子-1(TIMP-1)]水平、生命质量[慢性静脉功能不全问卷-14(CIVIQ-14)]评分、疼痛[视觉模拟评分法(VAS)]评分,以及并发症发生率。结果:治疗后3个月,观察组VCSS和CIVIQ-14评分均低于对照组,差异有统计学意义(P<0.05);治疗后3个月,观察组IL-2、TIMP-1水平均低于对照组,IL-10水平高于对照组,差异有统计学意义(P<0.05);治疗后1 d,观察组VAS评分低于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:超声引导下射频消融术联合聚多卡醇局部注射治疗下肢静脉曲张患者可降低VCSS评分、CIVIQ-14评分和VAS评分,改善血清学指标水平,效果优于单纯超声引导下聚多卡醇局部注射治疗。
Objective:To observe effects of ultrasound-guided radiofrequency ablation combined with local injection of Polydocanol in treatment of patients with varicose veins of lower extremities.Methods:A prospective study was conducted on 150 patients with varicose veins of lower extremities admitted to this hospital from January 2020 to January 2023.They were divided into observation group and control group according to the random number table method,75 cases in each group.The control group was treated with ultrasound-guided local injection of Polydocanol,while the observation group was treated with radiofrequency ablation on the basis of that of the control group.Both groups were followed up for 3 months after the treatment.The venous clinical symptom severity score(VCSS),the serological indicator levels[interleukin-2(IL-2),interleukin-10(IL-10),tissue inhibitor of matrix metalloproteinase-1(TIMP-1)],the quality of life[14 item-chronic venous disease quality-of-life questionnaire(CIVIQ-14)]score,the pain[visual analogue scale(VAS)]score,and the incidence of complications were compared between the two groups before and after the treatment.Results:After 3 months of treatment,the VCSS and CIVIQ-14 scores of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).3 months after the treatment,the levels of IL-2 and TIMP-1 in the observation group were lower than those in the control group,the level of IL-10 was higher than that in the control group,and the differences were statistically significant(P<0.05).1 day after the treatment,the VAS score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).However,there was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:Ultrasound-guided radiofrequency ablation combined with local injection of Polydocanol in the treatment of the patients with varicose veins of lower extremities can reduce the VC
