摘要
目的探讨高分辨率磁共振成像(MRI)联合表观弥散系数(ADC)对直肠癌新辅助化疗效果的评估价值。方法采用回顾性研究,收集2021年1月至2021年12月于驻马店市第一人民医院接受新辅助化疗后行根治性手术的81例直肠癌患者的临床资料。所有患者新辅助化疗前后均接受高分辨率MRI、MRI-DWI等检查,以肿瘤降期情况为疗效评估标准,以术后病理组织学结果为金标准,使用Kappa一致性检验高分辨率MRI联合ADC对直肠癌新辅助化疗的效果进行评价。结果81例患者中,病理组织学结果显示有效降期76例,无效降期5例。高分辨率MRI显示有效降期70例,无效降期11例,高分辨率MRI与病理组织学结果具有中等一致性(Kappa=0.454,P<0.001)。MRI-DWI检出有效降期72例,无效降期9例,与病理组织学结果具有中等一致性(Kappa=0.534,P<0.001)。高分辨率MRI联合ADC检出有效降期73例,无效降期8例,与病理组织学结果存在较强一致性(Kappa=0.750,P<0.001)。结论高分辨率MRI联合ADC对直肠癌新辅助化疗效果具有较高的评估价值。
Objective To investigate the value of high-resolution magnetic resonance imaging(MRI)combined with apparent diffusion coefficient(ADC)in evaluating effect of neoadjuvant chemotherapy for rectal cancer.Methods This study was a retrospective trail.The clinical data from 81 patients with rectal cancer who underwent radical surgery after neoadjuvant chemotherapy in the First People’s Hospital of Zhumadian from January 2021 to December 2021 were collected.All patients underwent high-resolution MRI,MRI-diffusion-weighted imaging(DWI)before and after neoadjuvant chemotherapy.The efficacy evaluation criteria were tumor progression,and the postoperative histopathological examination was the gold standard.Kappa consistency test was used to evaluate the value of high-resolution MRI combined with ADC in predicting treatment effect of neoadjuvant chemotherapy on rectal cancer.Results Among the 81 patients,the histopathological results showed that 76 cases had effective down-staging and 5 cases had ineffective down-staging.High-resolution MRI showed in 70 cases of down-staging and 11 cases of down-staging;high-resolution MRI and histopathological results showed moderate consistency(Kappa=0.454,P<0.001).MRI-DWI detected out 72 cases of effective down-staging and 9 cases of ineffective down-staging,with moderate consistency with histopathological results(Kappa=0.534,P<0.001).High-resolution MRI combined with ADC detected out 73 cases of down-staging and 8 cases of down-staging,with high consistency with the histopathological results(Kappa=0.750,P<0.001).Conclusions High-resolution MRI combined with ADC has high value in evaluating therapeutic effect of neoadjuvant chemotherapy on rectal cancer.
