摘要
目的分析腹腔镜辅助胃癌根治术对胃癌患者血清指标及预后的影响。方法本研究为回顾性分析。抽取2018年1月至2022年1月驻马店市第一人民医院收治的82例胃癌患者为研究对象,按照手术方式分为对照组(40例)和研究组(42例)。对照组采用开放手术,研究组采用腹腔镜辅助胃癌根治术。比较两组疼痛介质[P物质(SP)、前列腺素E2(PGE2)]、炎症因子[肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)、白细胞介素-6(IL-6)]、氧化应激指标[丙二醛(MDA)、血红素加氧酶-1(HO-1)、超氧化物歧化酶(SOD)]、肿瘤标志物[糖类抗原125(CA125)、糖类抗原199(CA199)、癌胚抗原(CEA)]水平及预后情况。结果术后,两组SP、PGE2、TNF-α、CRP、IL-6、MDA、HO-1水平均高于术前,但研究组低于对照组(P<0.05);术后,两组SOD水平均低于术前,但研究组高于对照组(P<0.05)。术后,研究组CA125、CA199、CEA水平低于对照组(P<0.05)。研究组2年内总病死率(9.52%,4/42)低于对照组(27.50%,11/40),P<0.05。结论腹腔镜辅助胃癌根治术治疗胃癌患者术后炎症及应激反应、疼痛程度较轻,肿瘤标志物水平降低较为明显,预后较佳。
Objective To analyze the effect of laparoscopic radical gastrectomy on serum indicators and prognosis in patients with gastric cancer.Methods This study was a retrospective trail.Eighty-two patients with gastric cancer treated in the First People’s Hospital of Zhumadian from January 2018 to January 2022 were selected as the study subjects,and they were divided into control group(40 cases)and study group(42 cases)according to surgical methods.The control group underwent open surgery,while the study group underwent laparoscopic radical gastrectomy for gastric cancer.The pain mediators,including substance P(SP)and prostaglandin E2(PGE2),inflammatory factors,including tumor necrosis factor-α(TNF-α),C-reactive protein(CRP)and interleukin-6(IL-6),oxidative stress indicators,including malondialdehyde(MDA)and heme oxygenase-1(HO-1)and superoxide dismutase(SOD),tumor markers,including carbohydrate antigen 125(CA125),carbohydrate antigen 199(CA199)and carcinoembryonic antigen(CEA),and prognosis were compared between the two groups.Results After surgery,the levels of SP,PGE2,TNF-α,CRP,IL-6,MDA and HO-1 in the two groups were all higher than those before surgery,but the levels of the above indicators in the study group were lower than those in the control group(P<0.05).After surgery,the SOD level in both groups were lower than those before surgery,but the SOD level in the study group was higher than that in the control group(P<0.05).After surgery,the levels of CA125,CA199,and CEA in the study group were lower than those in the control group(P<0.05).The total case fatality rate within 2 years in the study group(9.52%,4/42)was lower than that in the control group(27.50%,11/40),P<0.05.Conclusions After laparoscopic radical gastrectomy,the inflammation,stress response and pain degree in gastric cancer patients are less,while the levels of tumor markers decrease significantly,and the prognosis is better.
