摘要
目的:观察超声引导下肩袖间隙神经阻滞结合按摩治疗粘连期肩关节周围炎(简称肩周炎)的临床疗效。方法:选取2018年6月至2019年6月郑州市骨科医院门诊收治的粘连期肩周炎患者128例,按照随机数字表法分为治疗组和对照组,每组各64例。治疗组给予超声引导下神经阻滞结合按摩治疗,对照组给予按摩治疗。观察两组治疗前后疼痛程度[采用视觉模拟评分法(visual analogue score, VAS)评价]、肩关节功能[采用Neer肩关节功能评分表(以下简称Neer评分)评价]及临床疗效。结果:两组患者治疗后VAS评分均低于治疗前,且治疗组治疗后、治疗后3个月VAS评分低于对照组,差异有统计学意义(P<0.05)。两组患者治疗后Neer评分均高于治疗前,且治疗组治疗后、治疗后3个月Neer评分高于对照组,差异有统计学意义(P<0.05)。对照组有效率为92.1%,治疗组有效率为98.4%,治疗组有效率高于对照组,差异有统计学意义(P<0.05)。结论:超声引导下肩袖间隙神经阻滞结合按摩治疗粘连期肩周炎能减轻患者疼痛,改善肩关节功能。
Objective:To observe the clinical efficacy of ultrasound-guided rotator cuff space nerve block combined with massage on peri-arthritis of shoulder(referred to as shoulder periarthritis)during adhesive phase.Methods:A total of 128 patients with periarthritis of shoulder during the adhesive period admitted to the outpatient department of Zhengzhou Orthopaedic Hospital from June 2018 to June 2019 were selected and divided into the treatment group and the control group according to the random number table method,with 64 cases in each group.The treatment group was given ultrasound-guided nerve block combined with massage treatment,while the control group was given massage treatment.The pain degree[evaluated by visual analogue score(VAS)],shoulder joint function[evaluated by Neer shoulder joint function rating scale(hereinafter referred to as Neer score)] and clinical efficacy of the two groups before and after treatment were observed.Results:After treatment,the VAS scores of the two groups were lower than those before treatment,and the VAS scores of the treatment group were all lower than those of the control group after treatment and 3 months after treatment,with statis-tical significance(P<0.05).The Neer score after treatment was higher than that before treatment,and the Neer score after treatment and 3 months after treatment in the treatment group was higher than that in the control group respectively,with statistical significance(P<0.05).The effective rate of the control group was 92.1%and that of the treatment group was 98.4%,and the effective rate of the treatment group was higher than that of the control group,the dfference was statistically significant(P<0.05).Conclusion:Ultrasound-guided nerve block in rotator cuff space combined with massage can relieve pain and improve shoulder joint function in patients with shoulder periarthritis during adhesive phase.
